2 WEST EMPTY TRACK, WHILE TURNING THE TROLLEY POLE ON HIS LOCOMOTIVE, THE POLE BECAME ENTANGLED WITH A SIGNAL LIGHT PULL CORD CAUSING A SMALL METAL PULLY FOR THE PULL CORD TO STRIKE THE VICTIM IN THE MOUTH CUTTING HIS LIP & CHIPPING A TOOTH
WHILE OPERATING A LOCOMOTIVE AT THE FACE EMPTY SIDE TRACK THE VICTIM STOOD UP IN THE LOCOMOTIVE OPERATORS COMPARTMENT AND STRUCK HIS HEAD ON THE ROOF. HE WAS ATTEMPTING TO REALIGN THE TROLLEY POLE ON THE TROLLEY WIRES.
WHILE OPERATING A TRACK HAULAGE LOCOMOTIVE, VICTIM PASSED UNDER A CROSSING TROLLEY WIRE THAT WAS GUARDED WITH WOODEN BOARDS. THE VICTIM STRUCK HIS HEAD ON THE WOODEN GUARD JAMMING HIS HEAD DO WN ONTO HIS NECK. THIS ACCIDENT INVOLVED NO VIOLATIONS OF APPROVED RULES AND OR REGULATIONS. VICTIM WORKED UNTIL FIRST DAY MISSED 6/20.

THE JEEP JUMPED A T A TRACK JOINTS HOWEVER, NO UNSAFE TRACK WAS OBSERVED BY TRACKMAN AND FOREMAN WAS RESPONED TO THIS INCIDENT.
WHILE UNCOUPLING A MINE CAR AT THE ROTARY DUMP VICTIM WAS UNCOUPLING CAR WITH HIS RIGHT FOOT OF THE BUMPER STIRRUP THE ROTARY DUMP OPERATOR MOVED THE CARS AND WHEN THE SLACK CAME OUT OF THE CO UPLER IT CAUGHT THE VICTIMS RIGHT FOOT BETWEEN THE COUPLER OF THE FRAME OF TAE MINE CAR.
MAIN LINE ON EM1TY SIDE AT BOREHOLE, VICTIM REACHED IN BETWEEN AN 8 TONLOCOMOTIVE AND A STANDING FLAT CAR (WRECK TRUCK) TO REMOVE A SAFETY CHAIN. THE OCOMOTIVE MOVED AND THE VICTIM CAUGHT 2 FI FINGERS OF LEF7 HAND BETWEEN CHAIN AND LOCOMOTIVE COUPLER. THIS WAS IN VIOLATION OF SAFE WORK INSTRUCTIONS FOR THIS JOB
WHILE RIDING IN3AN OPEN TYPW PERSONNEL CARRIER,SAID CARRIER STRUCK A PARKED LOCOMOTIVE THAT WAS UNATTENDED AND SITTING ON THE MANLINE TRACK HAULAGE.THE FORCE OF THE IMPACT CAUSED THE VICTIM TO LEAVE HER SEAT3AND STRIKE HER KNEE/LEG ON THE FRAME OF THE CARRIER.IT WAS DETERMINED THIS WAS A RESTRICTED WORK CASE ON 08/24/87.

WHILE ATTEMPTING TO PLACE A LOADED 900 TON BARGE AGAINST A RIVER CELL A 11/2 INCH ROPE SUPPED AND STRUCK VICTIM IN LEFT ANKLE FRACTURING SAME.
WHILE CUTTING SUPPLY CARS IN YARD VICTIM WAS STRUCK IN RIGHT LEG BY TRIP WHICH WAS MOVED OPPOSITE OF INTENEDED DIRECTION.
WHILE ATTEMPTING TO UNCOUPLE A LOADED COAL CAR AT THE 19 1/2 TIPPLE VICTIM PLACED LEFT FOOT ATOP RAIL AT THE SAME TIME THE 19 1/2 TIPPLE STARTED TO DUMP AND THE TRIP OF CARS STARTED TO MOVE THE FRONT WHEEL OF A 8 WHEEL 18 TON LOADED COAL CAR RAN OVER HIS LEFT FOOT
WHILE ATTEMPTING TO REMOVE A SKID FROM A PARKED COAL TRIP VICTIM PINCHED HAND BETWEEN SKID AND RAIL CAUSING LACERATION OF RT HAND HE HAD JUST PUSHED TRIP OFF OF SKID REACHED DOWN FOR SKID SLAC K IN TRIP CAME BACK AND SKID CAUGHT HAND
WHILE ATTEMPTING TO RE-RAIL A OFF TRACK LOADED MINE CAR VICTIM WAS USING A BAR TO ALIGN THE WHEELS OF THE CAR WHEN THE CAR SHIFTED & PIVOTED THE WHEELS. THIS CAUSED THE BAR TO M OVE & CATCH VICTIMS HAND BETWEEN THE BAR & T HE COUPLER OF THE CAR.
WHILE OPERATING A SHUTTLE CAR, VICTIM HIT A HOLE IN HIS TRAMWAY AND STRUCK HIS RIGHT ELBOW ON FRAME OF SHUTTLE CAR. VICTIM CONTINUED TO WORK HIS REGULAR JOB UNTIL FIRST DUAY MISSED WORK WAS 10 -28-84 AND IT WAS NOTUNTIL DETERMINED ON 2-18-85 THAT THIS INJURY WAS DIRECT CAUSE OF NOT WORKING.
WHILE ATTEMPTING TO REALIGN A COUPLER ON A PIECE OF ROLLING HAULAGE EQUIPMENT (WRECK TRUCK) VICTIM CAUGHT AND PINCHED HIS SEFT FOOT BETWEEN THE COUPLER AND THE FRAME OF THE HAULAGE EQUIPMENT THIS INVOLVED NON-COMPLIANCE WITH JOB TASK PROCEDUTES FOR COUPLING MINE CARS METATARSEL BOOT WERE BEING WORN.
