Employee was coming up hill in fuel truck, when truck spun out, slid back 50 feet, then truck went up on spoil and laid over on it's passenger side.

Employee stated that front right-side tire went down causing employee to go in the ditch and scrap the highwall on the haul back to the parking lot area.

Employee was operating Caterpillar 777 Rock Truck and was headed back from the dump area through a narrow area in the roadway when another rock truck coming from the pit area also entered the narrow area. Employee hit the brake and caused the truck to slide toward the other truck and employee jerked the steering wheel in the other direction and went into an empty pond.

Employee was driving down haul road in personal vehicle when employee met an employee from the day shift crew leaving work early coming up the haul road. Employee coming down the haul road was in the center of the road striking the day shift's employee vehicle in the driver's side front.

Tandem Coal Truck driver was attempting to change to a higher gear on a slight decline. When driver changed gears, EE thought it had gone into gear but was actually in the neutral position. When the driver realized the truck was in the neutral position, the truck had gained speed causing the truck to runaway.

EE was operating 980M Caterpillar Frontend Loader cleaning up loose material in the pit area, when bucket became lodged in solid material. When EE went to back up the bucket became dislodged from the solid material and slammed EE down, causing the seat to bottom out on the seat frame.

Employee was operating 777 Rock Truck when they hit a hole in road jarring lower back and felt numbness in left leg.

Employee was heading down the hill in 785C Rock Truck after unloading, road was wet after raining, lost control, started going side ways and hit a rock wall and turned over.

Employee was traveling around a curve when he stated that it felt like the truck set down and it turned over onto his right side

Employee was sowing grass using the Honda 350 4-wheeler when he was going around a mud hole when the 4-wheeler hit a tree and turned over hurting his left side and shoulder. The injury date was 4-23-15, did not go to Doctor until 5-12-15. He was put off from work to do tests.

Was helping a mechanic on the Mack Water Truck, he was wiggling the steering wheel when it spun around and caught the lower part of his arm breaking it around the wrist.

Employee was driving a Volvo A40F truck when he hit a hole, jerked steering wheel hurting lower back and right leg.

Employee was cleaning coal with a 930 loader when his bucket caught on a sand rock and he twisted his back. He kept working with back pain thinking it may go away, but the pain kept getting worse. On 8/13/12 he thought he was having a kidney stone attack and went to the ER At Harlan ARH. He then called and said it was his back.

Employee was driving coal truck, when he went into the ditch causing the truck to turn over and the coffee thermos came up and hit the employee in the upper lip.

980 Loader turned over

Employee was dumping a load of rock when the truck started to turn over, he tried to let the truck bed down but the truck turned over. The truck flipped four times before coming to a rest on all 6 wheels.

Employee went to get out of truck, the door opened to easy and the employee fell out. Employee caught his foot between the steering wheel and the seat hurting his groin area and back.

Employee was descending down a coal haul road when he lost control of his coal truck and went over an embankment.

EE WAS PICKING UP SOME WIRE WHEN A PIECE OF COAL FELL FROM A CONVEYOR STRIKING HIS LITTLE FINGER.

EE WAS DRIVING HIS PICKUP TOWARDS THE FUEL TANKS WHEN HE LOST CONTROL IN A CURVE WITH LOOSE GRAVELS. THE TRUCK SKIDDED INTO A DITCH INJURING THE EE'S LITTLE FINGER.

EE WAS DRIVING TRUCK TOWARD LOAD AREA WHEN HE DROVE THE TRUCK INTO A DITCH (FILLED WITH SOUPY MUD), THE SUDDEN JERK THREWEE'S RIGHT SHOULDER INTO THE WINDOW POST.

EMPLOYEE WAS GETTING LOADED WHEN THE BOOM OF THE LOADER STRUCK THE TRUCK. EMPLOYEE WAS KNOCKED INTO DOOR HURTING SHOULDER.

EE WAS DRIVING THE MECHANICS TRUCK FROM PETERS KNOB TO ST CREEK WHEN HE HIT A BUMP AND FELT A SHARP PAIN IN HIS LOWER BACK.

LOADER WAS LOADING EMPLOYEE'S TRUCK WHEN A LARGE ROCK FELL FROM THE BUCKET JERKING THE EMPLOYEE'S BACK.

EMPLOYEE WAS BREAKING COAL WITH AN END LOADER WHEN THE BUCKET OF THE LOADER WENT THROUGH THE COAL, STRIKING THE BOTTOM AND HURTING EMPLOYEE'SBACK.

ROCK FELL FROM SHOT STRIKING LOADER CAB BUSTING SIDE GLASS. GLASS CUT EE'S ARM AND EE WAS COMPLAINE OF A SORE NECK.
