WORKER WAS ENTERING MINE ON PRESHIFT EXAM OPERATING GOLF GART WHEN BATTERY TERMINAL BURNED OFF WHICH CAUSED CART TO LOSE DYNAMIC BREAKING ON GRADE. WORKER FELT MECHANICAL BRAKES WERE NOT SLOWI NG HIMDOWN FAST ENOUGHT, SO HE RIBBED CART WHICHCAUSE IT TO ROLL OVER BRUISING HIS RIGHT HIP AND STRAINING HIS LOWER BACK. WORKED DID NOT MISS WORK BUT WAS PLACED ON RES DUTY ON 10-28-96.
WORKER WAS OPERATING A GOLF CAR ALONG AN INTAKE TRAVELWAY, SLOWLY LOOKIGN IN EACH X-CUT. FOR USABLE BELT STUCTURE WHERE THE HEIGHT VARIED FROM 40" TO 48" DUE TO UNEVEN ROOF AND BOTTOM CONDITI ONS AND HE ACCIDENTALLY STRUCK HEAD ON ROOF WHICH STOVED HIS NECK AND BACK. HE CONTINUED TO WORK FOR SEVERAL DAY, BUT WENT TO DR ON 8-15-96 WHERE HE WAS PLACED ON RESTICTION.
WORKER HAD BEEN HAVING PAIN IN HIS LOWER BACK FROM THE WORK ENVIRONMENT, BUT ON 6-22-96 HE ENCOUNTERED MORE PAIN THAN USUAL FROM RIDING ON HIS GOLF CART IN THER MINE AND OUT. WORKER DID NOT MI SS ANY WORK DUE TO THIS INCIDENT BUT RECEIVED A LOWER BACK ADJUSTMENT FROM THE CHIROPRACTOR ON 6-27-96 AND WAS PLACED ON RES WORK DUTY THEN.
EE WAS TRYING TO BRING A GOLF CART OUT OF THE MINE THAT HAD SEVERAL SPRING PLYS BROKEN AND HE ENCOUNTERED LOSS OF TRACTION ON A STEEP PORTION OF A GRADE. HE LET THE CART DRIFT SLOWLY BACKWARDS A FEW FEET WHEN THE SPRING SHACKLE BROKE CAUSING THE CART TOE ROLL OVER ON HIM IN DEEP MUD. THE WORKER RECEIVED A CONCUSSION, TORN LIGAMENTS, AND SPRAINED MUSCLES.
WORKER WAS ENTERING MINE ON GOLF CART WHEN HE LOST THE REVERSE TRAM DYNALE BRAKING SYSTEM. THE WORKER TRIED TO REPOSITION THE SELECTOR FOR 2 X CUTS WHILE RIDING HIS MECHANICAL BRAKE AND EVENTU ALLY DECIDED TO RIB THE CART WHICH RESULTED IN ALOWER BACK SPRAIN AND A POSSIBLE CRACKED RIGHT WRIST. DR ARE NOT SURE ABOUT WRIST. WORKER WAS PLACED ON RES WORK DUTY ON 5-20-96.
WAS DESCENDING A GRADE UG ON A GOLF CART WHNE IT BURNED A BATTERY TERMINAL LEAD OFF CAUSING IT TO LOSE ITS BRAKING SYSTEM. EE APPLIED MANUAL BRAKES AND TURNED INTO THE COAL RIB, CAUSING THE CA RT TO OVERTURN ON HIM. EE WALKED TO NEAREST MINE PHONE & WAS TAKEN TO HOSPITAL WHERE SLIGHT RIB FRACTURE WAS DISCOVERED.
WORKER WAS OPERATING PERSONEL CARRIER WHILE ENTERING THE MINE ON A PRE-SHIPT EXAM & AT THE TOP OF A STEEP GRADE HE APPRENTLY BUMPED THE REVERSE TRAM SELECTOR INTO NEUTRAL ACCIDENTALY. THIS CAU SED HIM TO LOSE REVERSE TRAM & HE IMMEDIATELY TURNED THE VEHICLE INTO THE RIB CAUSING IT TO ROLLON HIM. HE RECEIVED A BROKEN RIB, BRUSIED ANKLE,& BACK PAIN. 1ST DAY OFF WAS 1/5/96.
WORKER WAS A PASSENGER IN A GOLF CART DRIVEN BY EE COMING OUT OF THE MINE WHEN THE CART HIT A BUMP IN THE UNEVEN MINE BOTTOM WHICH JARRED THE WORKERS LOWER BACK WORKER RECEIVED A LOWER BACK AD JUSTMENT FROM A CHIROPRACTOR ON 08-28-95 NO RESTRICTED OR LOST TIME WAS INCURRED BY WORKER YET
BRAKES FAILED ON MANTRIP,GOLF CART. WORKER JUMPED OFF GULF CART, LANDING ON LEFT HAND TO BREAK FALL. WHEN HE LANDED ON LEFT HAND HE FELT PAIN IN LEFT SHOULDER. DIAGNOSIS SPRAIN LEFT SHOULDER. RELEASED FOR SELECTIVE DUTY ON 8-4-95. NO LOST DAYS WERE INCURRED.

WORKER IS RECEIVING LOWER BACK ADJUSTMENTS FROM CHIROPRA
YWORKER WAS DRIVING A GOLF CAR OUT OF THE MINE, HE SAID THE FOWARD TRAM CUT OUT AND THE CART STARTED BACKWARDS, HE APPLIED THE HYDRAULIC BRAKES THE CAR APPEARED TO SLIDE ON THE ICE, HE TRIED TO PUT THE CART INTO THE RIB TO STOP IT, WHEN HE DID IT ROLLED OVER ON HIM.

WORKER WAS HAULING CRIB BLOCKS INTO MINE ON SCOOP TRACTOR BATTERY END FIRST, AT 17 WALL THERE WERE STEEL BOOMS AND LEGS SET WITH WOODEN CRIBBING ON TOP, THE CLEARENCE IS LOWER THEN TRAVELING P RIOR TO POSSIBLY THE WORKER HAD THE BUCKET TOO HIGH AND CAUGHT ONE OF THE BEAMS KNOCKING CVRIB INTO KITCHEN OF SCOOP CAUSING CRUSHING INJURIES TO THE WORKER AS THE SCOOP KEPT MOVING BROKEN
WORKER WAS TRAMMING SCOOP ON TRAVEL WAY AND WAS IN THE PROCESS OF TURNING SCOOP ONTO A DOWN HILL GRADE. WHEN HE APPLIED HIS BRAKES THE BRAKE PISTONS BLEW SO HE QUICKLY SWERVED THE SCOOP BACK I NTO THE STRAIGHT AND ATTEMPTED TO SCRAPE THEIR RIB WITH IT SCOOP BUCKET TO STOP THE SCOOP. WHEN HE DID HTIS THE BUCKET CAUGHT RIB AND JARRED WRECK SHOULD.
WORKER WAS TRAMMING SCOOP FROM SECITON TO CHARGING AREA UP A STEEP SLIPERY SECITON OF TRAVELWAY WHILE BATTERIES WERE WEAK WHEN BREAKE RON SCOOP KICKED WHICH CAUSED HIM OT ROLL BACKWARDS ADN SL AM INTO COAL RIB JOLTIN GHIS BACK AND NECK. BRAKE PISTON ALSO BLEW. NO LOS TTIME, BUT RECEIVE DLOWER BACK ADJ.
DUE TO REAR SPRING BREAKING ON A GOLF CART MAN WAS RIDING ON THE REER OF THE CART TO APPLY ADDITIONAL TRACTION WHEN THE CART WOULD NOT MAKE IT AROUND 2 CLEUATED CURVE AT BOTTOM OF 3 MAIN HILL. MAN S FEET WERE HANGING OVER REAR TO CART, WHENANOTHER CART ATTEMPTED TO HELP PUSH THE MANS FEET WERE PINCHED BETWEEN THE BUMPTER OF THE CARTS.
MAN WAS USIGN AFORKLIFT TO LAOD 8' WOOD POST ON A SCOOP TRACTAR. DUE TO SNOW AND ICE ON THE POSTS, ONE LSIPPED OFF THE FORKS AND STRUCK THE MAN ON THE LEFT FOOT. MAN WAS HAND LOADING THE SC OOP AT THE TIME, FROM THE RAISED FORKS. MAN ICCURRED A FRACTURE TO THE LEFT FOOT. MAN DID NTO SEE A DR UNTIL 1-26-94. ON RESTRICTED DUTY FOR 2 DAYS, RELEASED FULL TIME 012994.
MAN WAS ONA GOLF CAR DESCENDING A GRADE IN THE AREA OF 3 MAIN WHEN THE BRAKES FAILED. MAN PUT CART IN REVERSE TRAM AND THIS ALSO FAILED. MAN DIRECTED CART INTO RIB. UPON STRIKING THE RIB THE THE CART ROLELD OVER ONTO THE MANPINNING HIM UNDERNEATH. MAN INCURRED A FRACTURE OF THE LEFT RIB AREA AND BRUISES OF THE RIGHT SHOULDER.

MEN WHERE USINGA SCOOP TO PULL OUT A SHUTTLE CAR THAT WAS STUCK. AFTER FREEING S/CAR. HE STIPPPED THE OPERATORS TO REMOVE THE CABLE THAT WAS ATTACHED BETWEEN THE EUQIPMENT. WHEN HE ATTEMPT ED TO REMOVE THE CABLE THE S/CAR DRIFTED AND THE LOOP IN THE CABLE WRAPPED AROUJND THE MAIN'S HAND.
MANW AS OPERATING A GOLF CART IN AN AREA OF THE EAST MAINS, #4 ENTRY. HEIGHT APPRXO 38 TO 40 INCHES. A BATTERY CHANGING UNIT LOCATED ON THE BACK OF THE CAR WAS SETTING UP TO HIHG TO CLEAR WH EN THE MANREACHED BACK TO MOVE THE CHANGING UNIT'S SIDE HE CAUGHT HSI HEAD AGAINST THE MINE ROOT CAUSING A SEVERE SPRAIN OF THE NECK AREA.
WHILE OPERATING A SHUTTLE CAR IN THE 10 LEFT SECTION THE INJURED MAN STRUCK AN OVERHEAD ROOF MST WITH THE CANOPY DISLODING ONE END OF THE MAT. THE LOOSE END ENTERED THE OPERATORS COMPARTMENT U NDER THE CANOPY AND STRUCK THE MAN IN THE CHEST CAUSING A COMPRESSION INJURY TO THE LEFT CHEST MINING HEIGHT IS APPROX. 42 INCHES IN THIS AREA.
MAN REPORTED ON 1-21-94 THAT HE HAD GONE TO A CHIROPRACTOR BECAUSE OF PAINS IN THE LOWER BACK. HE STATED HE FIRST STARTED TO EXPERIENCE THESE PAINS WHILE OPERATING A SHUTTLE CAR IN THE 10 LEFT SECTION. THIS SECTION IS LOW AND PITCHED. THE ONSET OF THESE PAINS OCCURRED DURING AUGUST OF OF '93 BUT HE DID NOT SEE A DOCTOR UNTIL THE ABOVE DATE. NO LOST WORK DAYS.
MAN WAS OPERATING AN ARTICULATING RAM CAR, WHEN HE MADE A 90 DEGREE TURN AT AN INTERSECTION HE PINCHED HIS RIGHT MIDDLE FINGER BETWEEN THE TWO SECTIONS OF THE RAM CAR.
VICTIM WAS WALKING BEHIND A UNA-HAULER THAT WAS TRANSPORTING A SMALL DISTRIBUTION BOX AND PULLING SEVERAL DEAD TRAILING CABLES BEHIND IT WHEN THE UNA-HAULER ENCOUNTERED A ROLL IN THE MINE BOTT OM WHICH CAUSED THE D-BOX TO CATCH ON THE MINE ROOF AND FLIP ONTO THE VICTIM'S LEFT HIP DISLOCATING IT. SEAM HEIGHT WAS 44"-48".
WORKER WAS OPERATING A SHUTTLE CAR INTHE 4 1/2 RT SECTION AND AFTER HE WAS FINISHED LOADING AT THE CONT. MINER HE TURNED AROUND IN HIS SEAT TO FACE HIS DIRECTION OF TRAVEL AND PINCHED A NERVE IN HIS NECK. HE LATER RECEIVED A BACK ADJUSTMENT FROM A CHIROPRACTOR AND MISSED ONE DAY.
OPERATING PERSONNEL CARRIER ON WAY OUT OF MINE WHEN SMALL PIECE OF DIRT CAME OFF TRAVELWAY INTO LEFT EYE. WORKER RUBBED EYE,DIRT SCRATCHED EYE CAUSING IRRITATION . LOST SAFETY GLASSES IN THE SECTION DURING SHIFT.

WORKER WAS A PASSENGER ON A HIGH PERSONNEL CARRIER COMING OUT OF THE MINE THE CARRIER ENTERED AN AREA THAT WAS LOWER DUE TO ADD ROOF SUPPORT AND WHEN HE BENT OVER FOR CLEARANCE HIS HELEMT AND CAPLIGHT FELL OFF WHICH IMPEDED HIS VISION AND HE RAISED UP TOO SOON AND STRUCK HIS FACE AND HEAD ON A STEEL H BEAM.

AS THEY ACCOMPLISHED THIS THE CAR STARTED TO DRIFT S 36638 FORWARD. INJURED QUICKLY PLACED HIS RIGHT FOOT ONT HE BRAKE PEDAL WHICH CAUSED HIS LEFT FOOT TO SLIP AND HE HIT HIS TAILBONE ON THE CAR SEAT.
WORKER WAS TRYING TO SLIDE A BROKEN DOWN GOLF CART OUT OF THE WAY OF TRAVEL IN AN AREA WHERE THE BOTTOM WAS WET AND SLIPPERY WHEN HIS FOOTING SLIPPED AND HE STRUCK THE FRONT OF HIS MOUTH ON TH E FRAME OF THE GOLF CAR CRAKCING 3 OF HIS FRONT TEETH.

THEY CAME UPON A LOWQER CLEARANCE AREA ON THE TRAVELWAY DUE TO ROOF SUPPORT THAT HAD BEEN SET THERE PREVIOUSLY. EMPLOYEE TRIED TO BEND LOWER TO THE RIGHT AND THE PROBLE POLE PARTIALLY STOPPED HIM FROM GETTING LOWE ENOUGH, HE
OPER SCOOP, BEGAN TO UNLOAD BAGS OF CE MENT ONE OF BUCKET JACKS FAILED BUCKET FELL ON L FOOT

THE SUTTLE CAR BUMPER ACCDIENTIALLY HOOKED ONTO A LOOP ON THE MINER WATER HOSE AND WHEN THIS HAPPENED THE WATER HOSE PULLED EMPLOYEES FEET OUT FROM UNDERNEATH HIM WHICH MADE HIM FALL ON HIS HE AD, NECK AND BACK.
EMPLOYEE WAS KNEELING DOWN AT THE TAIL SHAFT OF ONE OF THE SHUTTLE CARS.THE OPERATOR CAME OVER TO MOVE THE CAR AND DID NOT SEE HIM ON THE OPPOSITE SIDE OF THE CAR.WHEN THE OPERATOR MOVED THE C AR THE TIRE RAN OVER THE EMPLOYUEES RIGHT FOOT BENDING IT BACKWARDS.
EMPLOYEE WAS CLEANING ROADWAYS WITH SCOOP AND EITHER THE SCOOP BUCKET OR VIBRATIONS FROM THE SCOOP BUCKET DISLODGED A PIECE OF RIB WHICH STRUCK THE WORKER ON THE LEFT SHOULDER AND HAND HIS H AND WAS ON TOP OF THE SCOOP FRAME
