APPARENTLY HE HAD NOT LOCKED THE SWITCH HANDLE DOWN. THE LOCK MECHANISM IW WORKING PROPERLY. BRUISES & CONTUSIONS AROUND KNEE AREA.

TRUCK HIT HIGHWALL OF ROCKS.
EMPLOYEE WAS HAULING REFUSE UPHILL. HE RAN OVER A ROUGH AREA IN ROAD CAUSING HIM TO BE BOUNCED AGAINST DOOR OF TRUCK HE HIT AGAINST HIS LEFT RIB CAGE.
EE WAS OPERATING ENDLOADER LOADING RAILCARS. THE CONSTANT BOUNCING MOTION INJURED HIS LOWER BACK.

EMPLOYEE WAS HAULING REFUSE TO DUMP. ON A RETURN TRIP HE LOST CONTROL OF TRUCK DUE TO SLICK ROAD CONDITIONS. THE FRONT WHEELS SLID INTO DITCH, PITCHING OPERATOR FORWARD INTO DASH BAORD. THE BU STED HIS MOUTH & POSSIBLY SPRAINED HIS LEFT HAND.
