WHILE OPERATING HIS END DUMP TRUCK ON THE DUST CONTROL WATERED ROAD, THE INJURED SLID ON THE WET ROAD INTO THE BERM AND ROLLED THE TRUCK OVER. A SMALL LACERATION IN HIS HEAD REQUIRED 2 STITCHES TO CLOSE.

RETURNED TO WORK 9/4, WORKED 4 HOURS AND WENT HOME CLAIMING BACK PAIN WAS WORK RELATED, INJURY FROM DRIVING END DUMP TRUCK. INJURED WOKE UP ON 8/27 WITH BACKACHE, WORKED ALL DAY, WAS NOT SCHEDULED ON 8/30 & 8/31. OFF 9/1-9/3 THEN 9/5 AGAIN.
