Employee was loading coal truck with loader when he backed over the edge of the coal seam onto the pit floor, which was not planed, causing him to be jerked around in the cab.

DRIVER STATES THAT HE FELL ASLEEP AND DROVE INTO A SUMP HOLE ON HAUL ROAD. SEAT BELT BROKE AND HE LANDED ON FIRE EXTINGUISHER. DRIVER WAS TAKEN TO THE HOSPITAL BY AMBULANCE AND DIAGNOSED WITH CONTUSION OF RIGHT HIP AND LOWER BACK.

Belt bar came off mantrip, caught rib and hit left foot.

Employee was hauling rock in a 777D. As he was being loaded, the 992G operator dropped a rock into the truck and it rolled off hitting the side of the truck bed. Jarring the employee inside the truck.

Employee was running shuttle car - started around corner & hydraulic locked up causing car to hit rib.

EMPLOYEE WAS TRAMMING SHUTTLE CAR TO #1 ENTRY FROM FEEDER WHEN HE HIT A HOLE THROWING HIM INTO CANOPY.

Employee was cleaning roadways when the canopy of the scoop got caught on a bent bolt and it hit employee in face.

Employee had his hand between two sections of hauler. He steered hauler, mashing his middle and ring finger on right hand.

While tramming a unihauler, the canopy caught a 1/2 header hanging down which broke the canopy loose and it fell off striking employee in the head and neck. Employee did not miss any work until 11/29/2004.

Employee was putting starter fluid in forklift when forklift started and bumped him in the back.

ROCK TRUCK DRIVER TOLD FOREMAN AND SHIFT SUPT. THAT HE HIT ROUGH SPOT IN HAULROAD AND HURT HIS BACK. ROAD WAS WET, MUDDY, AND ROUGH. WEATHER-RAIN. HE WORKED THE REMAINDER OF SHIFT-1530-0030.

EMPLOYEE WAS REMOVING SPROCKET FROM THE TAILSHAFT ON THE MINER. IT WAS BEING PULLED BY A CHAIN WITH AN ENDLOADER. EMPLOYEE APPROACHED THE SPROCKET AND THE CHAIN BROKE, STRIKING EMPLOYEE ON THE LEFT SIDE OF HIS FACE.

DRIVER OF TRUCK #02 A 1998 MACK R888 TANDEM COAL TRUCK WAS DESCENDING FROM THE NORTH PIT WITH A LOAD OF COAL APPROACHING H-18 AREA OF COAL HAUL ROAD WHEN HE STRUCK A POT HOLE IN THE ROAD WITH SAID TRUCK. JARRED HIS BACK. CAUSED PAIN TO LEFTLEG.
