Mostrando los 47The mirror of a customer's truck contacted employee's hand while they were providing direction from portable steps, causing a hand contusion.
Employee reported feeling lightheaded and dizzy. Seen at clinic and cleared for work but referred to an ear specialist later for noncommunicable condition resulting in swelling in employee's ear. Employee reported a possible connection to a loud noise employee heard earlier in the shift as a loader filled an empty hopper.
Employee was operating a front end loader, they leaned on right arm to adjust seat, hit a pothole and jammed their right arm up into their neck resulting in right shoulder pain. The incident was reported on 12/13/23.
Employee was inspecting tracking on a bagging belt. When they squatted down to inspect the belt from beneath they brought a hand up and the belt caught their glove pulling their thumb between the belt and a pulley.
A plant support specialist was operating a CAT 980M loader and separating larger boulders from the stockpile. EE drove with the bucket in the air and as they dumped the bucket, the loader tipped and laid over on the right side. The operator made contact with the inside of the cab causing pain to arm and knee.
Employee was operating a forklift on uneven ground causing the seat to bounce which resulted in a sore back that was reported the next day.
EE was driving pickup truck on site when an object (dust/debris) entered EE's eye when EE blinked. Physician removed item, and provided medication to prevent infection.
EE was cleaning material from hydrator discharge chute using metal shovel/scraping tool to push material down the chute into screw conveyer. The end of the shovel was caught by the auger and pinched EE's hands between the tool handle and side of the chute. EE was treated for bruising and strained ligaments in EE's right thumb, placed on restricted duty with follow-up scheduled.
EE was driving a front-end loader through the Quarry - he ran over a pot-hole and stated the loader seat bottomed out and injured his back.
6/12/14 EE reported back strain from oper. FEL. EE did not see Phys. until 7/1/14-ordered off of work for 2 wk. Follow up visit on 7/15/2014 EE was cleared for light duty starting 7/22/2014. Phys. has not determined if work related or existing non-work related injury from summer/2013 and again 3/2014 -incident being treated as reportable pending diagnosis.
Dump truck driver was driving with box in the air for too long when the box struck the overhead conveyor, driver was thrown from truck onto ground & truck rolled to a stop approx 1/4 mile later. Driver has some compressed & cracked vertebra but is out of hospital & walking. Conveyor has bent pieces but was retrained & working within 30 min. Hoist & subframe on truck were bent.
7/12/2013 EE was driving truck on site, he drove over a bumpy portion of the mine road causing his elbow to strike the arm rest. EE reported injury on 7/15/2013 EE saw phys. on 7/18/2013-CT scan ordered shows bone chip on elbow. Follow-up reading sched. 8/6/2013 to determine if work related or an existing injury. Item being treated as reportable pending verification by phys.
EE informed MGT of back pain on 2/12/2013. EE had been running a skidsteer loader/water truck-did not see physician until 2/15/2013, an MRI was scheduled for diagnosis. 2/21/2013 Phys.diag. was a bulging/ruptured disk in lower back-placed on light/sedentary duty. Item being treated as work related unless/until directed otherwise by phys.
Employee was closing hatches on OTR customer truck when the driver drove off with EE still on trailer. EE was forced up against the side rails of the safety platform but hung on and kept from falling to ground. EE suffered sprains to neck/back, contusions to arms, legs and back. EE prescribed RX for pain in the p.m, and phys. therapy 3x wk. No lost time/restricted duty.
Employee removed guard to adjust conveyor take-up pulley. While cleaning mud off the pulley the wrench was caught in the roller and his finger was pulled into and pinched by the frame.
Employee called from home complaining of leg pain possibly from repetetive motion.
Driving forklift, turned too sharp causing forklift to roll to right side, put right leg out to catch self and the forklift rollover cage fell on his right foot.
7694 & 7697 haul trucks met at the big hill on the Roberts Road. 7694 (being driven by EE) was going down the hill with a load. 7697 was coming up the hill, empty. When they met, EE applied the brakes. EE's truck went to the left, up a steep bank and stopped. Then the load shifted and it rolled over onto the right side.
THE HAUL TRUCK DRIVER CAME OFF A SMALL HILL WITH TOO MUCH SPEED IT SEEMS AND HE LOST CONTROL GOING UP THE NEXT HILL & HIT A BERM WITH THE LEFT TIRES & ROLLED THE LOADED HAUL TRUCK ONTO THE RT. SIDE.
EMPLOYEE WAS BACKING UP A RAMP ON BASE COURSE STOCKPILE. HE BACKED UP GRADUALLY ON BERM ON BLIND SIDE OF TRUCK CAUSING THE TRUCK TO GO OFF THE PILE.
SHIFTING GEARS REPETITIVE USE EE WAS OPERATING A 980C LOADER HE SAID HE WAS SHIFTING THE LOADER INTO ANOTHER GEAR AND FELT A POP IN HIS WRISTS
UPON LEAVING WORK HIT LOOSE GRAVEL CAUSING REAR END TO FISH TAIL AND OVER CORRECTED SENDING JEEP INTO NORTH DITCH- SOMEHOW HER FOOT HIT THE ACCELERATOR AND CONTINUED THROUGH PASTURE, WENT AIRB ORNE UPON HITTING EMBANKMENT-LANDED AND THEN ROLLED 1.5 TIMES STOPPING AND FLIPPING ON THE DRIVER'S SIDE.
THE SCREEN CHUTE WAS PLUGGED WITH BIG ROCKS & EEWAS UNLODGING THE BRIDGED ROCK & A BIG ROCK THATHE HAD ALREADY FREED ONTO THE CONVEYOR BELT STARTED TUMBLING BACKWARDS & HIT HIS HAND.
EE WAS OPERATING THE WATER TURCK. HE SAID HE WAS TURNING TO THE FINE GRIND PLANT ON THE NORTH SIDE OF THE WREHOUSE, HIT A HOLE WITH THE TRUCK AND HE HEARD HIS BACK POP. AFTERWARDS, HE HAD A PA IN IN HIS BACK AND COULD NOT MOVE WITHOUT IT HURTING. I HAD EE GO HOME AT APPROX. 1430.
DRIVING TRUCK CROSSING RAILROAD TRACKS WAS HIT BY A TRAIN ON DRIVER'S SIDE.
EE WAS DRIVING A FORKLIFT TO THE TRUCK SHOP FROM THE LIME PLANT. ANOTHER TRUCK PASSED HIM, CAUSING A GREAT DEAL OF DUST. SHORTLY AFTER THAT EE NOTICED AN IRRITATION IN HIS RT EYE. A PHYSICIAN AT BLACK HILLS REG EYE INST REMOVED A PARTICLE OF LIME FROM HIS EYE.
EQUIPMENT WAS SUSPENDED (HOPPER)-BY A 988 LOADER-THE HOPPER SLID ACROSS THE TRAILER AND SWEPT THE EMPLOYEE OFF HIS FEET.HE STRUCK THE GROUND HEAD FIRST.THIS RESULTED IN A CHEST CONTUSION.
EE ALLEGES TO HAVE BEEN STRUCK ON THE RIGHT HIP BY A ROCK THAT HAD VIBRATED FROM THE GRIZZLY HE WAS WORKING UNDER. HE SAID THE ROCK STRUCK THE MACHINE'S FRAMEWORK FIRST AND THEN GLANCED OFF ST RIKING HIS HIP. THE NAMED WITNESS DID NOT SEE THE ROCK STRIKE EE.
THE OPERATOR WAS WALKING AROUND THE REAREND OF THE CAT 988B LOADER HE HAD BEEN OPERATING WHEN SOMETHING BLEW INTO HIS LEFT EYE. THE LOADER WAS IDLING EE DOESN'T KNOW IF THE ENGINE COOLING FAN BLEW SOMETHING OR IF IT WAS FROM THE WIND. REASON FOR LATE SUBMISSION EE DID NOT INITIALLY EXPERIENCE ANY PROBLEMS WITH HIS EYE. OVER A PERIOD OF TIME COMPLICATIONS DEVELOPED AND HE SAW A PHYS
EMPLOYEE WAS HAULING LARGE OVERBURDEN ROCK FROM THE QUARRY PIT. AT 10AM NOTICED LOWER BACK PAIN AND LEGS GOING TO SLEEP.
EE COMPLAINING OF NUMBNESS IN HANDS & FOREARMS, DID NOT GIVE SPECIFIC DATE OF INJURY.
WAS TAKING CHAIN OFF ROTARY FEEDER ON MILL, THE SLACK OF THE CHAIN CANTAINED TENION AND CAUGHT THE LEFT THUMB BETWEEN CHAIN & SPROCKET, TEARING TIP OF THE LEFT THUMB OFF.
EMPLOYEE WAS MOVING RAILROAD CARS WHEN THE FRONT ENDLOADER HIT A BUMP CAUSING HIM TO LOSE HIS BALANCE THE SAFETY BELT STOPPED HIS FALL PULLING SOME MUSCLES IN HIS LOWER BACK
HE WENT TO VISUALLY INSPECT A VIBRATING FEEDER AND A SURGE OF MATERIAL CAME BURSTING OUT FROM PAN STRIKING THE RIDE SIDE OF THE FACE.HE WAS WEARING SAFETY GLASSES.
IT IS ALLEGED THAT EMPL WAS GOING IN REVERSE IN FRONT END LOADER & HE WAS TWISTED & HIT SOME BUMPS & HE PULLED OR TORE MUSCLE IN BACK.
EMPL TRAINING TEMP.DRIVER ON HAUL PACK WHILE LEAVING FOR VACATION.HE WAS RIDING ON SIDE BY OPER SEAT WHICH HAS NO PADDING.HE HAS A COMPRESSED DISC WHICH IS PUTTING PRESSURE ON NERVE IN LOWER B ACK.SITTING ON METAL SHROUD FOR AIR CONDITIONER UNIT-HAUL PACK HAS ONLY ONE SEAT.
STOCKPILING 1" BASE COARSE WITH 769 QUARRY TRUCK (35 TON CAT OFF HIGHWAY TRUCK.) QUARRY TRUCK BACKED UP ON STOCK PILE OF BASE COARSE, BACKING TOO CLOSE TO EDGE, WHEN THE STOCK PILE BANK GAVE A WAY CAUSING THE QUARRY TRUCK TO FLIP OVER.
IT IS ALLEGED THAT THE EMPLOYEE WAS DRIVING A DUMP TRUCK. WHILE DRIVING HE BACKED UP TO THE DUMP AT THE EDGE AND WHILE DUMPING THE REASR WHEELD SEATTLED CAUSING THE WHOLE TRUCK TO GO OVER BAC KWARDS. THE EMPLOYEE SUFFERED A PUNTURE WOUND NEAR LEFT TEMPLE AND NUSSLE BRUISES TO MID BACK.

IT IS ALLEDGED 7HAT THE EMPLOYEE WAS DRIVING TRACTOR TRAILER UNIT HAULING A SCALPER & SCREEN UNIT THE LOAD WAS TOP HEAVY & THE UNIT LAID OVER CAUSING THE EMPLOYEE TO KBE JARRED AROUND ON THE CAB EMPLOYEE SU0FERED BRUISED ANKLE & KNEE & CONCUSSION TO HEAD
THE EMPLOYEE WA8 HELPING PUT A PUMP IN THE BUNKER WHEN HE LOST HIS BALANCE AND PUT HIS FOOT IN THE FRAME AND THE EMPLOYEE RUNNING THE BOBCAT ACCIDENTALY DROPPED THE BOBCAT ARM ON HIS FOOT CAUS ING SEVERAL FRA7TURES OF THE RIGHT BIG TOE.
WHEN EMPL WAS R6NNING FRONT-END LOADER ,JARRING AND VIBRATING OF LOADER CAUSED BACK STRAIN OF NECK AND UPPER BACK IN T-4 AREA.
EMPLOYEE WAS PLACING BELT DRESSING ON CONVEYOR BELT WITH A STICK THAT STUCK TO THE BELT BECAUSE HE DID NOT LET GO OF THE STICK HIS ARM WAS DRAGGED BACK AGAINST THE TAIL PULEY GUARD CAUSING HIS ARM TO FRACTURE
IT IS A ALLEGED THAT EMPLOYEE WAS WORKING AS A GROUNDMAN KEEPING MATERIAL CLEARED OUT UNDER CONVEYOR BELTS THAT RUN AT THE PLANT.
EMP WAS LOADING ANFO ONTO PICKUP HE PULLED PICKUP AHEAD TO CLOSE DOOR ON THE VAN THE PICKUP ROLLED BACK PINNING EMP BETWEEN THE PICKUP AND THE VAN
MR. REINER WAS DRIVING ON THE QUARRY FRONTAGE ROAD & MADE A RIGHT TURN TOWARDS THE SCALE HOUSE & WAS CROSSING THE R/R TRACKS AT WHICH TIME HE WAS STRUCK BY R/R ENGINE #4512 OON THE LEFT SIDE OF THE WATER WAGON. REINER WAS THROWN CLEAR OF THE WATER WAGON & LANDED INCHES OF THE R/R TRACKS.
