Operator stated that while the rock truck was being loaded, dust/debris had blown into the open window and into left eye causing irritation. The on site medic diagnosed that their cornea was scratched. Antibiotics were prescribed to prevent infection.

EE was lifting the cab of a bobcat skid steer. As the cab was lifted up, the door fell closed, catching the tip of EE's left middle finger. This resulted in a fracture to the tip of the finger requiring sutures.

Mud on transmission linkage prevented shifting from park. Driver exited the running pickup, set chocks, then went under pickup. While cleaning linkage, driver accidentally separated the shift cable from the transmission linkage. The operator unknowingly moved linkage from park to reverse. The wheel chocks were not set tight against tire allowing contact with driver at right flank.

Operator lost control of a fully loaded Caterpillar 777 F Model haul truck while attempting to make a left hand turn into a dump area. When applying the brakes ee lost traction and the equipment rolled over onto it's right side.

Grinding Operator reached through a gap between two guards to remove a plugged spray bar at the 2B conveyor. While attempting to remove the spray bar, it contacted the conveyor and wrapped around the bend pulley. The spray bar was ripped from the operator's hand resulting in a partial amputation of the operator's right ring finger tip.

While operating a loader, employee notified the Supervisor of excessive exhaust fumes in the cab. Mechanic discovered the loader hydraulic breather for the steering oil tank exceeded service life, and also found a small hole in an exhaust hose.

Operator of 777 haul truck ( #7714 ) was hauling from 925-332 shot to waste dump #5. When the operator was coming in for lunch felt a sharp pain in lower back and couldn't bend over or twist. The Supervisor asked if there was anything that happened that could of caused the back pain and the operator said not sure what happened.

HAUL TRUCK DRIVER WAS TRAVELING UP THE EXHAUST TO THE GRIZZLY. AT THE INTERSECTION AT THE WEST RAMP EE HIT SOME POT HOLES IN THE ROADWAY CAUSING EE's HARDHAT TO FALL OFF EE's HEAD AND BOUNCING EE's HEAD OFF THE ROOF OF THE HAUL TRUCK. THIS RESULTED IN THE OPERATOR RECEIVING A MINOR CUT TO EE's HEAD.

The driver was heading from the Anarraaq met station site back to RDM when the snowmachine broke through thin ice atop overflow. The snowmobile began to zig-zag, he lost control of the machine and it tipped over. When the driver got up, he had pain in his right elbow and the passenger his right knee and forearm.

A 777 haul truck driver complained of neck soreness at the end of his shift. The operator was unable to attribute the soreness to any one event, however did not have soreness at the beginning of the shift.

Took the Lull forklift into the building to get a motor off one of the shelves and as he was backing he broke the window with a piece of rebar sticking out of the side of the building.

Employee was shutting door while in the cab of the sky track and his left thumb was in between the door and the dash as the door shut smashing tip of thumb

A loader operator backing up with a bucket of ore and hit the back of the haul truck bed with the side of the loader bucket. When the loader hit the truck the driver was shaken / jarrred which caused the injury.

While unloading onto compactor garbage truck, the dumpster became unlatched and swung around striking helper. Helper sustained laceration.

There was a missing vent on the A/C vents upon which the employee stuck his hand up into and struck the fan that was rotating. This caused a laceration on the left middle finger of the employee.

The injury occurred when the employee was riding underground seated at the rear of the mancarrier. As the mancarrier was travelling it hit a bump on the road causing the employee to bounce up off the seat and strike his head (hard hat) against the roof of the mancarrier.

Employee had his thumb pinched on the hinged portion of a haul truck door.

Operator was delivering two connex's of sand to the sand filter tent with the 8233 tractor trailer and 40' high deck. He was going to stop, shifting from 2nd gear to reverse when he lost his brakes and could not get the truck stopped. He drove the truck to the bottom of the hill and rolled it over before inpacting the hill.

While loading a ore truck, a chunk of ore fell out of the bucket on the right side. The operator did not see the chunk and ran over it, severly jarring the machine, causing a fast side-to-side movement of the neck and head of operator, straining the neck from impact.

Employee was operating a Haul Truck when he drove over a pot hole in the road, which in turn had bottomed out the seat he was sitting on.

PT CUT HIS RT. HAND ON A CONVEYOR BELT WHILE WORKING AT THE PORT THIS EVENING.

The ee was riding out of the mine in the mancarrier when the injury occurred. The ee stated he injured his hip when the mancarrier hit a pothole in the roadway, causing him to bounce hard on the seat.

HURT HIS LOWER BACK LAST NIGHT WHEN HIS SEAT BOTTOMED OUT IN HIS VEHICLE.

INJURED EE AND EQUIPMENT OPERATOR WERE IN THE PROCESS OF HANGING FIBER OPTIC CABLE, WITH INJUREDEE IN MANBASKET ABOUT 8' OFF THE GROUND. OPERATOR REACHED TO CLEAR CABLE FROM MAN CARRIER AND INADVERTANTLY BUMPED THE DUMP CONTROL LEVER. THE SAFETY CHAIN CAME LOOSE, ALLOWING THE BASKET TO DROP TO THE GROUND WHICH INJURED EMPLOYEE IN IT.

EE WAS OPERATING FRONT END LOADER. DUE TO THE ROUGH RIDE OF THE LOADER, THE EE FELT A TIGHTNESS IN HIS LOWER BACK.

EE WAS REMOVING A PIECE OF LUMBER FROM THE SCRAP BIN INSIDE THE MILL AND WAS SQUEEZED BETWEEN A ROLL OF BELTING BEING MOVED BY A FORKLIFT AND A STATIONARY SCRAP BIN.

WHILE ATTACHING THE CABLE TO THE REAR HITCH OF THE WALDON, THE WALDON DRIVER BACKED UP, BRIEFLY PINNING EMPLOYEE BETWEEN THE MACHINE AND THE I BEAM OF THE CSB.

EMPLOYEE WAS ASSISTING TO CHANGE OUT A 10X10 PUMP CASING. AS THE CASING WAS BEING LIFTED OVERHEAD THE SLING BROKE AND THE CASING FELL APPROX.10' TO THE FLOOR. IT STRUCK A SPOOL PIECE AND DEFLECTED AND STRUCK AND PINNED EMPLOYEE. CASINGWEIGHS 1700LBS.

EE WAS USING A SNOWMACHINE WHILE COLLECTING WATER SAMPLES THE MACHINE BECAME STUCK & HE HURTHIS BACK WHILE TRYING TO GET IT UNSTUCK

EMPLOYEE JUST STARTED TO WALK AWAY FROM THE CONTROL ROOM, WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK. HE HAD BEEN USING A WASHDOWN HOSE EARLIER.

EMPLOYEE WAS OPERATING A MANLIFT DRIVING DOWN A RAMP WHEN THE MANLIFT HIT THE ROAD BERM, CAUSINGIT TO TIP OVER. THE EMPLOYEE IN THE BASKET FELL APPROX. 8 FEET, LANDING ON HIS LEFT SHOULDER. HE SAW DOCTOR ON 07/02/99 THEN RETURNED TO WORK. HESAW DOCTOR AGAIN ON 11/04/99 AND HIS SHOULDER WAS OPERATED ON. FIRST DAY MISSED FROM WORK WAS 11/05/99.

TRAVELING TO DRILL SITE ON SNOW MACHINE. EE WAS A PASSENGER AND STRUCK HIS JAW ON DRIVER SHOULDER WHEN THEY HIT A BUMP

EE WAS RIDING A SNOW MACHINE TO TAKE ENVIRONMENTAL WATER SAMPLES. SKI CAUGHT ON A HUMP IN THE TUNDRA. HANDLE BAR JAMMED HIS LEFT WRIST. EE SAW A DR ON REGULAR R & R 10-22-98, AND RETURNED TO W ORK (LT DUTY) AT THE END OF HIS REGULAR R & R 10-28-98.

EE WAS REPOSITIONING A ROOF SHEET ON A CONVEYOR. IN ORDER TO CONNECT A CABLE TO THE TOP OF THE SHEET THE CONVEYOR WAS STARTED WHILE THE EE WAS IN THE PATH OF TRAVEL. THE PADDLES ON THE CONVEYO R CONTACTED THE TOP OF HIS FEET & CRACKED THE BONE.

MANLIFT APPROX. 120' IN THE AIR THAT HAD GOTTEN HUNG UP ON THE BOLTS OF A BEAM SPLICE. WHILE THEY WERE TRYING TO FREE THE MANLIFT IT JERKED FREE, MOVED SOME DISTANCE AND CAUGHT THE EE'S HAND B ETWEEN THE RAIL ON THE LIFT AND A STEEL BRACE. THIS RESULTED IN THE LOSS OF HIS RIGHT MIDDLE FINGERTIP AND THE LACERATION OF HIS RIGHT INDEX FINGER.

EMPLOYEE STATED THAT WHILE HELPING TO MOVE A CEMENT BARRIER, THE LOADER STRUCK THE BARRIER, CAUSING IT TO MOVE BACKWARDS, WEDGING HIS FOOT BETWEEN THE BARRIER AND THE GROUND.

DROVE 966 LOADER W/BUCKET TO MOVE A FALLEN OUTRIGGER PAD FRAM A 3900 MANITOWAC CRANE. SET BUCKET DOWN TO BAD PAD AND TOE WAS UNDER BUCKET.

EMPLOYEE IS COMPLAINING FROM LOWER BACK PAIN DU;E TO DRIVING A HAUL TRUCK. ALLEGEDLY HIT A CHUG HOLE IN JAN 1993 & HIS BACK GRADUALLY GOT TO THE POINT THAT HE COULD NO LONGER DRIVE THE TRUCK O N 7/14/93. EMPLOYEE WAS SENT TO THE DOCTOR, WHO GAVE HIM 2 DAYS OFF & 14 DAYS LIGHT DUTY DUE TO "LOW BACK PAIN, NEURALGIA". DID NOT REPORT THE ACCIDENT UNTIL 7/12/93.

EMPLOYEE WENT IN BETWEEN TWO STORAGE CONDXES. HE WAS GOING TO THE BATHROOM. ANOTHER EMPLOYEE WHOT WAS OPERATING A V-900 FORK LIFT, MADE A PICK OF ANOTHER CONEX. THAT PUSHE IN A 2ND CONEX PINCH ING THE EMPLOYEE BETWEEN THE 2ND & 3RD CONEX. V-900 OPERATOR WAS UNAWARE THAT THE PERSON HAD GONE BETWEEN CONEXES.

OPER SCOOP TRAM DID NOT HAVE FOOT INSIDE OF CAB R TOE STRUCK BEAM

EMPLOYEE STATED THAT WHILE RIDING ON THE BACK OF A LOADED ORE TRAIN, A CHUTE HIT A ROCK IN THE LAST CAR CAUSING THE ROCK TO SMASH THE EMPLOYEES RIGHT HAND.

THE AIR COMPRESSOR THAT SUPPLIES AIR TO THE BRAKES FAILED THE BOOM TRUCK ROLLED OVER AT A CORNER 1 1/2 TIMES.

EMPL STATED THAT WHEN ORE TRAIN HE WAS DRIVING CAME TOGETHER WITH LOADED WASTE TRAIN THAT WAS TRAVELING IN OPPOSITE DIRECTION. E HIT HEAD ON FIRST ORE CAR & RECEIVED CUT THAT REQUIRES STITCHES .

THE SPADE PART OF A SHOVEL WAS IN THE TAIL PULLEY OF A CONVEYOR BELT WHILE IT WAS RUNNING.THE EMPLOYEE WAS WATCHING IT TO SEE IF IT WAS GOING TO WORK ITS WAY OUT.I SOMEHOW WEDGED JUST RIGHT CA USING IT TO FLY OUT HITTING THE EMPLOYEE IN THE FACE CAUSING LACERATIONS TO THE NOSE AND LEFT SIDE OF FACE.

DOING PRESHIFT LIME IS ADDED SOME DROPPED OVER END OF FILTER IN HIS EYES

EMPLOYEE WAS MOVING BOULDERS TO CLEAR A PATH FOR MUCKING AS HE WAS TRAMMING THE LOAD A BOULDER FELL OUT OF THE BUCKET CAUSING THE FRONT END LOADER TO BUCK JOLTING THE EMPLOYEE CAUSING LOWER BA CK STRAIN

LARGE BOULDER WAS IN BANK WHERE WORK WAS BEING PERFORMED. BUCKET TEETH WAS PLACED UNDER BOULDER & PRESSURE WAS APPLIED. TEETH SLIPPED OFF BOULDER CASUING LOADER TO BOUNCE. THIS CASUED WHIPLASH MOTION TO NECK OF EMPL. DR SUGGESTED FEW DAYS REST WITH FULL DUTY RELEASE DATED 9/4.

WET HAUL ROADS AND "S" CURVES. OPERATOR DOWN SHIFTED AND HAUL TRUCK SKIDDED INTO SAFETY BERM AND TILTED ON ITS SIDE. CERVICAL RIGHT SHOULDER AND RIGHT FOOT. DECREASE IN MOTOR FUNCTION AND SENS ATION TO THESE AREAS.

EMPLOYEE WAS DRIVING A DUMP TRUCK OVER ROUGH TRRRAIN.THE TRUCK BOUNCED CAUSING THE EMPLOYEE TO BUMP HIS RIGHT KNEE.OVER THE NEXT TWO WEEKS THE KNEE BEAGN TO SWELL.ON O8-02-90 WE SENT INJURED T O THE DOCTOR.THE DOCTOR DRAINED THE FLUID OFF THE KNEE.
