Mostrando los 6FEL operator at the crusher was in the process of positioning the bucket over the side of Komatsu truck when EE hit the lever to come down slamming the bucket onto the side of the truck. FEL operator was talking on the 2 way radio to tell another truck to go get material from the plant when the accident happened. Operator having the mic caused EE to push it too far forward.

TWO EMPLOYEES WERE ATTEMPTING TO CHANGE THE V-BELTS ON THE #2 PRODUCT CONVEYOR DRIVE UNIT. ONE EE DISCONNECTED THE TURNBUCKLE FROM THE DIRVE UNIT. THE UNIT ROTATED FORWARD AND FORCED HIS FACE INTO THE FRAMEWORK OF THE BALLAST WASH SCREEN.
