Mostrando los 5Attempting to move a portable litter fence. He stopped forklift with the forks under side of the fence & exited the forklift. With forklift running, he went between the cab & lifting mechanism of the forks to chain the fence to the fork cage. He stepped backwards against the control lever & mechanism moved backward, pinning him between cage & the roof of the cab.
HE WAS CLEANING OFF A BELT SCRAPPER ON A HEAD WHEEL WHEN THE BAR SLIPPED THROUGH PAST THE SCRAPPER AND WAS CAUGHT BETWEEN THE BELT HEAD WHEEL & PULLEY. THIS PULLED HIS GLOVED HAND INTO THE BEL T. THE BELT WAS MOVING AT AN IDLE SPEED WHEN THE INCIDENT OCCURRED.
EE WAS OPERATING THE FRONT END LOADER AND BEGAN HAVING DISCOMFORT IN HIS BACK. HE CONTINUED WORKING. ON 11-4 HE REPORTED THE RPOBLEM TO OEPRATIONS MANAGER. ON 11-5 HE SAID HE WAS STILL HAVING LOWER BACK PAIN AND SLIGHT PAIN IN HIS LEGS AND WAS TAKEN TO DR. TO BE CHECKED.
ON 11/16/98 APPROX 9AM EE REPORTED TO QUARRY MASTER HE HURT HIS BACK 3 WEEKS AGO. HE SAID WHILE DRIVING TRUCK WHEN THE LOADER DUMPED ROCK INTO THE BODY OF HIS TRUCK IT JARRED HIS BACK. EE DID NOT REPORT IT IMMEDIATELY BECAUSE HE THOUGHT IT WOULD BE OK. UPON TALKING TO EE HE SAID THAT IT REALLY DID NOT HURT UNTIL THE MORNING OF 11/16/98
