Employee was standing beside a scoop bucket in an intersection loading belt onto a flat car when the employee slipped and fell. As the employee fell, hand went between the scoop bucket and the ram. The employee suffered lacerations on right hand after the ram smashed hand against the scoop bucket.

Employee was standing near the engine compartment of a 928 Front End Loader when they felt the equipment move. The employee jumped off of the 928 Front End Loader and injured the left knee, right shoulder, and forearm.

Employee was riding in track mounted mantrip when EE's foot was caught between the mantrip and a scoop axle laying beside the mine track.

Employee was cleaning belt line using a rake when the rake came into contact with a moving belt and caused the rake to strike the employee in the right side of the employee's face. Initially the CT Scan showed no fractures and employee returned to work on employee's next regular shift & worked until On 2/22/2016 another CT Scan showed a fracture in employee's right jaw.

Employee was riding on top the #31 mantrip coming off of the 9L section at the end of the shift. The trolley pole came off of the wire, struck the mine roof, then broke into. One of the jagged edges of the broken pole struck the employee on the right hand between the middle and ring fingers causing a laceration that required 5 stitches.

Injured was tramming scope when he hit a hole in the bottom throwing him into the canopy of the scoop. Causing pain in his neck. He finished the shift and saw the PA on site.

Injured employee was a passenger on #25 bus and spurred into 9G to allow supply motors to pass, when the supply motor hit the 9G switch, the motor veered into 9G, striking the #25 bus and injuring a miner.

Employee was operating a scoop with an interlocking double door. The safety pin was not installed correctly into the hasp and the doors opened while the scoop was in motion and struck the employee in the lower back.

Employee was transporting supplies to section on a motor trip, when he struck his head in low top and felt pain in his neck.

Employee was traveling as a passenger in track ride facing the direction of travel on West Mains track. At #142 break the employee struck a roof bolt causing the employee's head and neck to recoil. The employee was transported to the local hospital.

While unloading supplies, employee was struck by a scoop and bruised his right leg.

The maintenance crew was installing a planetary on the #71 Continuous Miner and utilizing a Scoop on the section to push the planetary into place. The Scoop Operator announced he was going to pull the injector back and as he did so, the injured employee had his thumb between the Scoop's blade and the injector.

Employee was traveling in a mantrip and as he reached up to open the D-rail switch at #32, he injured his right arm.

Employee was riding in the inby end of a open ended track vehicle when the vehicle struck a track stopping device (dog)causing the vehicle to come to an abrupt stop.

Employee suffered a lower back sprain when the track vehicle he was on was struck by a mantrip. The first day he missed work due to the injury was 1/20/2011.

A supply locomotive struck a track jeep on 8Q track with three employees sitting on the jeep. One of the employees suffered a contusion to the left elbow.

Employee pulled back on handbrake on mantrip and felt pain in his lower back. The employee did not miss any work until 4/5/2010.

Employee reached for trolley pole that had came off of the trolley wire when he felt pain in his back.

Employee was struck in the right hip by a lifting jack bar that fell through a drain hole on the mantrip while it was in motion. Employee didn't miss any work until 4/05/2010.

Employee had his hand resting on the back of the scoop blade when the operator of the scoop inadvertently pulled in the ram and mashed the employees right hand causing a fracture to the right ring finger.

Injured was struck by a slate bar that was hanging out of the side of a mantrip and came in contact with a wooden crib that had been set beside the track for roof support. As the slate bar contacted the crib, it swung around and struck the injured in the lower part of his left leg just below the shin causing a fracture to his fibula.

Injured was struck on the right foot by a rail that a scoop hit when tramming in the injured work area. Injured was transported to the hospital where he was diagnosed with a broken right big toe.

Injured was tramming shuttle car to the face when he hit a hole in the roadway causing his head to strike the canopy injuring his neck.

Injured was tramming shuttle car to the miner, when he hit a hole in the roadway, causing his head to come in contact with the canopy and injurying his neck.

Injured was operating a scoop and while tramming, injured jammed his head against the scoop canopy.

He was told to wait until the motor stopped, before he lined the draw heads up. The motorman pulled up 4' and stopped. The motorman told injured that he would pull up again and wait until he stopped before he lined the draw heads up. He then heard injured holler. employees have been contacted about working around moving equipment.

Injured was operating a scoop while pushing a disabled scoop to the charger. The scoop he was operating slipped off the disabled scoop after he had ran up ontop of it causing him to hit the canopy when it fell off. Injured is taking treatments for a compressed disc in his neck.

Injured was riding in portal bus #33. His right knee was outside of the bus and struck a fire valve along the West Mains track. Causing a contusion to the right knee.

The injured was preparing to unload roof bolting supplies when the scoop blade was lowered down on his left foot.

Injured was tramming a portal bus through the outby set of double doors when the trolley pole came off of the wire and swung around and struck him on the right side of the head. The trolley pole limiter was not in use.

Injured was scooping in the #2 entry, rt. rib. The scoop blade was back under the curtain on the rt. side of the scoop. When he backed up, the curtain hooked on the scoop, he was attempting to free the curtain while backing out and caught his hand between the scoop and the rib.

Injured was cleaning an area around a belt junction when his shovel got caught between a roller and the belt, twisting his wrist.

Injured walked up beside of a tramming scoop without letting the operator know that he was there. As he turn to flag the operator his left foot was ran over by the scoop tire.

Injured was tramming a scoop up to pick up some rock dust. When he went around a corner of a block he put his left hand outside of the scoop and in between the hinge point of the scoop just outside the operators deck. As he turned the two halves of the scoop came together catching his finger.

Injured was sitting on the edge of a supply car and the tire rolled up on his left leg resulting in a fracture.

Injured leaned over to try and locate an oil leak under the scoop. Injured instructed the scoop operator to turn on the scoop. When he did he pulled back on the ejector jack which caught the injured's fingers between the scoop blade and the ejector blade.

Injured was shaking a rock off of the blade of the scoop. He had not properly shut the 2 piece door. The back door swung open allowing the other part of the door to open up striking the steering jack which in turn swung around and struck the injured in the shoulder.

Mantrip that injured was riding in was struck in the rear by another mantrip causing injured to strike his head on the back of the mantrip compartment causing him to strain his neck and back.

Injured and co-worker had been building cribs in the #3 entry on the head of the longwall. Scoop operator was returning with a load of cribs. Employee was sitting on chain pillar rib just inby #53 break. The operator unknowingly trammed by him and ran over both of his feet. Operator heard injured call out and stopped tramming the scoop.

EMPLOYEE WAS HAULING MONO-RAIL ON THE OUTBY END OF A PORTAL BUS. MONORAIL CAUGHT A CHAIN HANGING DOWN FROM THE TOP. THIS CAUSED THE RAIL TO COME IN THE DECK OF THE PORTAL BUS STRIKING THE INJURED IN THE LEFT LEG.

AS EE WAS TRAMMING DOWN THE ENTRY, THE BATTERY HAULER HE WAS OPERATING (#2) BROKE IN HALF, THROWING EE AGAINST THE CANOPY, CAUSING HIS NECK TO HURT.

EE WALKED AROUND THE END OF A PARKED BATTERY HAULER WHILE AN EE WAS OPERATING THE EJECTOR BLADE. THIS WAS TO LINE UP BRACKET HOLES FOR PINNING THE EJECTOR INTO PLACE SO THE HAULER COULD BE USE D AS A MANTRIP. THE TAILGATE AUTOMATICALLY OPENS TO TOWARD THE MINE FLOOR WHEN THE EJECTOR IS OPERATED. THE TAILGATE CAME DOWN ON HER FOOT.

EE REPORTED THAT AFTER OPERATING THE SECTION SCOOP THAT HIS KNEE WAS HURTING.
