Employee was traveling to the crusher with a load of shot rock when EE failed to maintain control of the truck EE was operating. The truck traveled over a berm and struck a pillar in the underground mine. The employee was taken to a hospital for treatment.

Mostrando los 8Employee was diagnosing an issue with the swing operation of the manlift basket while in the basket. The employee leaned over the control panel while moving the basket causing the employee to strike head on the mine roof & the controls of the manlift. The employee was seen in the ER & given stitches for a laceration to the chin. The employee was released to light duty the same day.
Bed of truck came up after dumping load and bed hit ceiling. This is the second report being completed I have the initial report completed on file too.
At about 9:30 AM employee helped get a production sample off a conveyor belt. This was done the same way that our samples are always collected. At that time the employee felt fine. He continued cleanup around the plant in the skid steer. (His normal job) Around 11:00 he started having back pains when he hit bumps while operating the skid steer
EE returning to loader for a load turning around a pillar while downshifting and drove through low spot causing wheel to jerk as he was turning causing left hand to separate and strike door frame of truck.
EE was holding onto aftermarket steering knob, when wheel jerk occurred, the force caused knob to roll over and become flat with steering wheel. As wheel came around, and with EE still holding onto knob, his left hand was smashed between knob and inside of door frame, breaking a bone above left pinky finger.
On the 29th of Sept employee was in a manlift with co-worker loading faces. Co-worker was running the controls as they were going up the face. As they neared the top the operator didn't take into consideration that injured is a little taller & on the high side. He raised the lift too high hitting injured's head and injuring his neck as a result.
Employee was scaling when a rock fell in front of the scaling machine. A piece of the rock came down the scaling arm and through the glass. He received a small cut on his leg from the glass. The cut required on stitch.
