Trying to put motor back on the track with the forklift. The forklift slipped out making the forklift bounce up and down hurting his lower back
employee saw that the scoop was possibly going to catch the lifeline, he approached the scoop on the operators side without notifying the operator. As he grab the lifeline the scoop started to tram in reverse, he slipped toward the scoop and his left foot went under the bucket. The operator could not see the employee and the scoop bucket came down on the employees' foot.
Was loading roof bolter & the shuttle car was trammed toward him knocking him to the ground pinning his left arm above his head. NOTE: Suspect fraud, see attached statements from men that worked with him. When mine started laying off, he alleged injury on 7/30/13, but did not miss until 9/19/13, almost 2 months after the alleged injury! Mine laid off contractors 12/4/13. Job ended.
EE was tramming a shuttle car toward the miner, hit hole in the roadway and felt sharp pain in his back.
Unhooked chain to let vehicle leave property. Chain caught on vehicle and pulled it through hand and cut finger.
EE alleged that he was injured while driving a scoop to the battery station. EE said he hit a hole causing him to bump his head on the canopy of the scoop. EE worked the entire 9 hour shift.
This report is being filed to abate citation #8128500. Employee was in the process of cutting off a piece of metal at 3B tailpiece when a small piece of rock fell from the discharge roller and struck him over top of the eye resulting in restricted duty work.
EE was pulling a big rock off top of a fall with a chain. The chain broke and the scoop went down to the ground bouncing his head and neck into the canopy.
Employee was backing the Mac-8 up to let a scoop go by. While he was backing up, his hand was inside the steering wheel. The wheel hit the rail causing the steering wheel to hit his left hand cutting the little finger. He got three stitches in it.
EE was riding on the scoop (He should not have been there), his foot slipped into the point (scoop part that swivels).
Supply motor was pulling the car up and the supply car caught the back of his right foot. It twisted his ankle and chipped a small bone in the heel.
While riding mantrip out of slope, motor stalled causing mantrip to roll out of control hitting wall.
EE was riding outside on mantrip when brakes failed as he was toping out. Mantrip rolled back down slope, wrecking on bottom striking solid wall.

EE was checking back tire on scoop. Operator, never knew he was there. Scoop rolled back pinching back of rt foot. This EE started missing work 4/20/07.
Rib rolled out on track and man-bus hit rib roll. The rib wasn't all the way across the track, just a corner was on track. EE had his arm behind a co-worker holding onto canopy post, when bus hit; this caused a piece of coal to fly up and cut EE's fingers.
EMPLOYEE WAS CROSSING S/C CABLES AND PUT HIS FOOT ON IT TO CROSS. REEL TOOK UP THROWING HIM ON HIS LEFT HIP & BACK.
