Supervisor was in the loader picking up on the fifth wheel base to take pressure off of the shaft to remove the tail pulley stand base. Employee figured they could use the 1/4 bolt on the one side of the shaft for leverage to remove the shaft from the base. As they picked up with the loader to remove the pressure it smashed employee's finger between the bolt head and stand base.

Mostrando los 8Employee was driving loader 23-1042 moving rock up and down the pile. As employee was coming down ramp and struck pipe in ground and was thrown forward in cab striking chest/stomach on steering wheel.
Working out of a boat hanging power cord on float frames next to the dredge pipe. Front of boat slipped under pipeline, allowing forward movement of the boat, pinning person between metal bench seat and pipeline.
On May 23rd, 2012 employee radioed his superintendent that he was having chest pains & trouble breathing. He asked to be taken to the emergency room, which he was. There he was given oral nitroglycerin and morphine. The super stayed until girlfriend arrived. This point on were told by EE this was personal med problem and not WC. On 6/15/12 his attorney told us was work related.
As box was reaching max angle ee felt truck began to lean to drivers side. Immediately began dropping the box. Truck laid over on its driver side door when the box was down to about half way. Hit knee on the door. Went to ER to be checked & returned to work w/o restriction. EE return to DR on 5/31/11 with knee pain. Crutches & immobilizer restriction.
Employee was backed up to a dump area with a load of lime. When the haul box was raised to dump, the truck lifted to the left and went over on its side.
EE was backing haul truck up the stockpile ramp to dump load when the ramp gave slightly causing the haul truck to lay over on its side.
While dumping wet spoil material hauled from off site, truck tipped on its side. Due to fact material was stuck inside dump and driver was on sloped surface.
