There were some large rocks on the dumping berm. When the loader operator cleaned the rocks off the operator pushed the berm over the dump area. After the truck driver got another load the driver backed up to dump and didnt know the berm was out until the driver felt the wheel drop over the dump leading to driver tipping over the truck.
