Employee was replacing a drive sprocket on a excavator. While attempting to install the sprocket fell off of the forklift and landed on ankle. Took employee to the emergency room and had x rays taken. Results were no broken bones or torn ligaments. Ankle is fine just some swelling and bruising.

Employee states cleaning muck off the table and adjusting easier skirt with a hammer while belt was running. Hammer slipped and went into roller and took EE's hand in. Damages broken bones in right hand and swelling of the arm which lead to skin graphing.
Haul Truck down shifted after hitting bump while driver was reaching for their lunch box causing employee to feel stinger in lower back.
Mechanic was going to fuel drill at the bottom of east ramp when EE lost the brakes and rode truck down to bottom of pit where the truck flipped onto its left side.
Truck operator reported that EE was having back pain and difficulty breathing after hitting a rough spot in the road.
Shovel operator made contact with bed of haul truck, jolting the haul truck driver.
Employee was hit in the ankle when steel rail was bumped by forklift and slipped off wear metal.
Another contractor's truck was backing up and his rear bumper struck our worker's right leg/hip. Worker was taken to local hospital for x-rays/tests. All x-rays/tests came back clear. No broken bones or injuries sustained, only bruising. Worker was released to full work duty on June 29, 2015.
