EE initially said it was related to haul road and it is rough. EE later clarified that on 06/24 EE was driving the road and it was bumpy and the air seat bottomed out. EE stated was slammed to the bottom causing back pain. EE noted was worried about a compression type injury. EE did not seek treatment or report until 8/27/2020.
Was in the loader, thinks this occurred on Wednesday 06/10/20, EE hit the corner of a rock with the bucket and it jarred and tweaked EE's low back.

Operator was preparing to flip cutting edge on skid steer bucket, bolt started to spin in hole so operator tried to push pin back in square hole. Spinning bolt cut EE's left index finger resulting in 5 stitches. Operator was wearing gloves.
Driving haul truck when steering went out and braced for crash. The brakes were able to stop haul truck to a safe stop. Bracing for crash caused strain to both arms and right foot.
Employee did not stand clear of object when he signaled the boom truck operator to lift up. Resulting in torn skin on two fingers even though he was wearing proper PPE (Gloves)
Off-hwy end-dump truck veered off roadway onto the 8'+ shoulder, struck a buried drainage pipe, traveled up the steep slope adjacent to the shoulder, struck a boulder (approx 5' in dia.) & power pole, turned over onto its driver's side & came to rest on the side of the vehicle with the driver's side of the bed of the truck resting on the boulder.
Employee was operating a CAT End Dump, hauling up a slope when the load shifted to the back causing the front wheels to come off the ground. The load then dumped out and as the truck came back down the employee struck his head causing lacerations on both his lip and right eyelid.
WEATHER DRY AND CLEAR. NO ROADWAY OBSTRUCTIONS TO DRIVER'S VIEW. NO APPARENT TRUCK DEFECTS. NO APPARENT ISSUES WITH FEEDER LAYOUT APPROACH. INJURED WORKER MADE EYE CONTACT WITH TRUCK HAULAGE DRIVER EXITING FEEDER AS REQUIRED. ACCIDENT MAY HAVE RESULTED FROM DRIVER INATTENTION. EXACT IMPAIRMENT UNDETERMINED. DAMAGE TO BLOCK WALL REPAIRED.
WHILE RUNNING LOADER EE HIT BUMP CAUSING INJURY TO BACK AND LEFT LEG. REPORT NOT FILED TIMELY DUE TO LATE RECEIPT OF CLAIM.
EE WAS IN MACK TRUCK BEING LOADED BY A LOADER. WHEN THE LOADER WENT TO DUMP A LOAD OF G ROCK INTO THE DUMP TRUCK, THE WETNESS OF THE MATERIAL CAUSED IT TO COME OUT ALL AT ONCE. THE IMPACT OF THE WET MATERIAL CAUSED THE EE TO GET BOUNCED AROUND INSIDE THE CAB OF THE TRUCK HITTING HER HEAD AGAINST THE DOOR. CAUSATION: POSSIBLY LOADING WET MATERIAL TOO FAST.
