Mostrando los 7Employee was walking back to work station from lunch. EE was struck on hard hat by a rock while walking under the 3 division jaw belt. EE felt pain in neck and upper back. EE was taken to med center for evaluation.
Employee was operating a CAT 972M Rubber Tire Loader when EE felt pain in back. EE then reported this to immediate supervisor, who took EE to the Med Center to be evaluated. There was not a specific activity that triggered the back pain. EE was released back to work with restrictions. The employee has been a Loader Operator at the facility for 28 years.
While conducting an inspection/cleaning of the loader bucket, the operator had not completely lowered the bucket to the ground. The hydraulics slowly lowered while parked, causing the bucket to pinch the operators toes on left foot between the bucket and the ground.
They were lifting the bed of an R85 dump truck with the skip loader when it slipped and the bed of the truck came down full force. Employee was sitting in the seat in the cab of the truck. The truck jumped and the bed hit bottom throwing him up and all over.
EE WAS GOING INTO THE PLANT DRIVEWAY TO HAUL AWAY MATERIAL FROM THE PLANT. HE RAN INTO SIDE OF PLANT
TWO TRUCK DRIVERS WERE PLAYING TOPS ON THE LOADING PAD AND WERE OUT OF THEIR TRUCKS, DISOBEYING OUR COMPANY REGULATIONS, FAILED TO FOLLOW RULES ON SIGN ENTERING THE COMPANY. FORKLIFT DRIVER WA S PULLING OUT THE PALLET, TRYING TO AVOID RIPPING ANY OTHER PALLETS, HE WAS KEEPING AN EYE IN FRONT OF HIM, THEN TURNED HIS HEAD BACK AT THE SAME TIME HE HIT DRIVER.DROVE OVER TOES,LEG CAUGHT
