Worker was getting rocks out of tail pulley using an air hose. The tip of the air wand got clipped by metal gear which forcefully hit the wand and the top of worker's hand into the equipment above.

A rock came off of the feeder area and hit the person behind the ear right under EE's hard hat. Person reported to foreman at the time but did not request to be seen by a doctor

Cleaning under jaw crusher. When loader dumped load, rock fell over side - striking shoulder and head - causing bruising.

CHANGING BELTS AND CLEANING AROUND END PULLY. EE PUT ARM BETWEEN BELT WHEN CONVEYOR WAS STARTED.

STUCK HAND IN RETURN ROLLER AND ARM BECAME STUCK IN BELT. RIGHT ARM SUFFERED EXTENSIVE CONTUSION AND ABRASIONS NO FRACTURE.

WORKER WAS GREASING CONVEYOR GLOVE GOT CAUGHT AND PULLED HAND TOWARD OR ROLLER.

WHILE OPERATING A LOADER, SOME FORM OF AIRBORN PARTICLE ENTERED THROUGH THE OPEN CAB WINDOW STRIKING THE EMPLOYEE IN HIS LEFT EYE. SOME CORNEAL ABRASION RESULTED.
