Employee was cleaning up around main crushing plant with a skid steer. Went to dump the full skid steer bucket and a 7" rock fell out of the back of the bucket, striking the main frame of skid steer, and then fell into the cab, striking the employee's right knee cap.

Employee states on 6/15/20, while operating our haul truck, employee felt "muscle spasms", but continued doing job. On 6/22/20 employee went to the emergency room to get checked out after still experiencing muscle spasms in left ribs.

Employee was operating haul trk when he hit an obsticle in road, jarring the haul truck and causing the pulled muscle in lower back.

HIT BUMP BOUNCED IN SEAT SEAT BELT WAS ON. INJURY CRACKED RIB COLLAPSED LUNG. WE BELIEVE RIB INJURY OCCURRED OFF JOB WITNESSES REPORT SEEING INJURED EMPL BEFORE START OF WORK. WE HAVE FILED RE PORT IN CONJUCTION WITH W/C.
