EE was filling 50lb bags at MCC. EE noticed a bag was positioned wrong and went over to fix bag on conveyor. EE didn't shut conveyor off and went over to grab bag and reposition it. While doing so EE hit hand on conveyor #2 bracket and cut hand and received a small pinky bone fracture.

Didn't go to Doctor until 11-30-11.(No medical treatment)Thought he would be o.k. 3-28-12 still having problems with his shoulder. Sent to Co. Clinic on 4-6-12, received P.T. Claim became recordable.
Employee was driving the service truck as he was going up the road to the scales. He looked over at the 9.5 crane, and then he blacked out causing him to hit the fuel tanker. EE was not injured.
Employee was helping to move the plant. Employee was unhooking chains off the beam and the loader backed up and hit the beam causing the beam to fall over and falling on employee's left leg causing a sprain to his left ankle and a contusion/strain to his left calf. He is restricted to sitting work only.
EMPLOYEE WAS INSIDE LOADER PULLING RAIL CARS. HE TURNED THE FAN ON AND WHEN HE DID DUST AND DIRT BLEW INTO HIS LEFT EYE. HE RECEIVED A PRESCRIPTION AND SURGERY TO REMOVE ALL THE FOREIGN BODY.
EMPLOYEE WAS OPERATING THE BOBCAT FOR 4-5 HOURS AND WAS JARRED & JOLTED AROUND. EMPLOYEE WAS STRIFF WHEN HE GOT OFF THE BOBCAT. EMPLOYEE STATED HE HAD TO HAVE HELP TO GET OUT OF BED THE NEXT M ORNING. EMPLOYEE IS RESTRICTED TO LIMITED LIFTING.
WHILE HOOKING UP A BUCKET TO #121 SKID LOADER, EMPLOYEE WAS HOLDING UP BUCKET PINS WITH HIS FOOTAND WHILE ADJUSTING BUCKET HE WENT THE WRONG WAYWITH THE LEVER AND BUCKET PINCHED HIS FOOT AGAIN ST FRAME OF LOADER. HE WAS GIVEN A CAST TO WEAR AT WORK BUT HAD NO RESTRICTIONS.
EMPLOYEE WAS IN HOPPER TO CLEAN IT OUT. OTHER EMPLOYEES MOTIONED TO START THE BELT. THE LOADER OPERATOR THOUGHT THEY WERE TELLING HIM TO DUMP THE LOAD OF PIT RUN MATERIALS IN HOPPER. AS MATERI AL WAS DUMPED ON EMPLOYEE, HIS HARD HAT WAS KNOCKED OFF AND HIS HEAD WAS CUT BY THE PIT RUN MATERIAL. EMPLOYEE RECEIVED STITCHES.
EMPLOYEE WAS CLEANIGN UP ROCKS AROUND NEW #9 CONVEYOR. WHEN ONE OF THE ROCKS HE HAD THROWN ONTO THE CONVEYOR FELL BELOW THE BOTTOM OF CONVEYOR BELTS. EMPLOYEE PROCEEDED TOR EMOVE THE GUARD A ND REACHE DINTO TRY AND RETRIEVE THE ROCK. HIS RIGHT ARM BECAME CAUGHT IN TEH PINCH POINT OF HT ETAIL PULLEY, RESULTIN GIN HIS RIGHT ARM BEING TORN FROM HIS SHOULDER.
EMPLOYEE WAS DRIVING HIS VEHICLE TO GET PARTS FOR THE SUPT. AS EMPLOYEE PULLED OUT FROM THE RAMP, A DRIVER OF VEH. F2 RAN A LIGHT & HIT EMPLOYEE'S TRUCK ON THE LEFT DOOR. EMPLOYEE WAS EXPERIEN CING DISCOMFORT IN HIS LEFT SHOULDER.
EMPLOYEE WAS PICKING CLAY OFF CONVEYOR BELT CAUSING OVER USE OF HAND.
EMPL REACHED INTO TOP OF CONVEYOR BELT TO PULL ROCK OFF BELT & JAMMED HIS HAND ON ROCK.
EMPLOYEE WAS OP9RATING LOADER CLAIMS THE SUN BLINDED HIM & COULDNOT SEE THE OTHER LOADER COMING TOWARDS HIM & RAN INTO IT THE IMPACT CAUSED HIM TO GO FORWARD & HIT HIS LEG
