Employee unchoking hopper, locked out wrong discharge rotary valve. Reached into port, fingers pinched between valve flights and housing. Amputated left middle fingertip to nail and ring fingertip to cuticle. No surgery required. Will result in lost time and restricted duty.

Mostrando los 10EE was on the property of the Stockertown Quarry and needed to pass a cement truck that was parked. As EE was passing the cement truck the driver of that truck pulled rapidly away from parking spot thereby sideswiping EE's truck & pushing it to the right. Truck damaged & EE's wrist hurt.
Employee was operating his loader and when moving the shift lever he felt something "pop" in his left shoulder.

WHILE BACKING UP TO LOADER, REAR TIRE SUNK INTO THE MUD CAUSING TRUCK TO HIT THE LOADER ARMS. TRUCK JERKED AND EE WAS THROWN AROUND IN THE CAB. OPERATOR DID NOT GET THROWN OUT OF SEAT, BUT H ELD ONTO THE STEERING WHEEL AND THIS CAUSED THE SHOULDER INJURY.
USING HIGH REACH TO REPAIR HEAT TRACE ON WATER PIPES. HAD RIGHT HAND ON CONTROLS OF HIGH REACH AND LEFT HAND ON RAILING WHEN RAISING BASKET INTO PIPES. PINCHED LEFT HAND BETWEEN PIPES AND BASK ET.
SEARCHING FOR KEYS TO BOBCAT, WHEN SAFETY BAR FELL AND HIT BACK OF HEAD. WORK ORDER FOR REPAIR OF SAFETY BAR WRITTEN 12/99. CLARKLIFT WAS WAITING FOR PARTS. EE WAS UNAWARE OF PROBLEM. POOR COM MUNICATIONS CONTRIBUTED TO THE INJURY.
