Miner drove a loader across feeder #1 when material had been removed. The plant operator had triggered feeder #1 and failed to communicate to the loader operator. The miner was injured as the wheels on the loader dropped into the feeder, causing miner to experience multiple body strains.

WHILE CHECKING K2001 TRACKING, THE EE STOOD ON THE SW SIDE OF THE BELT. HE REACHED WITH HIS LEFT HAND IN BETWEEN TWO ROLLERS TO GET A ROCK WHILE THE BELT WAS RUNNING CAUSING L ARM TO FRACTURE.
THE EE WAS STANDING ON THE SCREEN WHILE THEY WERE LIFTING A CROSS OVER CONVEYOR BELT. HE PUT HISFOOT ON THE CONVEYOR TO TIGHTEN THE COMEALONG AND THE SUPPORT BRACKET WELD BROKE. THE CONVEYOR SWUNG OUT & HE LOST HIS BALANCE & FELL. HE LAND ED ON HIS RIGHT SIDE ON THE SUPPORT BRACKET. HE FELL APPROX 13 FT
EMPLOYEE WAS STANDING BETWEEN AN OIL STORAGE TANK & FRONT OF A FLATBED TRUCK & WHEN THE TRUCK WAS MOVED A FEW FEET A STEERING WHEEL ON THE TRUCK STRUCK ANEMBANKMENT & THE UNSECURED TANK SLIPPE D FORWARD & PINNED THE INJURED EMPLOYEE AGAINST THE FRONT OF THE TRUCK
EMPLOYEE STATES THAT WHILE HE WAS SHOVELING UP SPILT MATERIAL AROUND A PORTABLE COLLECTOR CONVEYOR BELT, THE SHOVEL HANDLE GOT CAUGHT IN THE BELT AND THE SHOVEL STRUCK HIM ABOVE HIS LEFT EYE.
EMPL TRYING TO STOP SAND LEAK IN DROP BOX CHUTE ON #1040 SHAKER SCREEN.THE CONVEYOR WAS RUNNING AT THE TIME AND AS HE PUSHED ON THE BELTING DEFLECTOR HIS HAD WAS CAUGHT WITH BELTING BY CONVEYO R BELT HEAD PULLEY AND WAS PULLED IN BETWEEN PULLEY AND BEARING BLOCK.HE WAS ABLE TO PULL ARM BOACK OUT OF THE ENTANGLEMENT.
1. FAILURE OF DRIVER TO FOLLOW INSTRUCTIONS 2. TRUCK BACKED TO EDGE AT AN ANGLE. 3. EMPLOYEE HAS EYE SIGHT PROBLEM. 4. BERM WAS UNDERMINED IN THIS AREA. 5. DRIVER NOT COMPLETELY AWARE OF J OB HAZARD. END DUMP DAMAGED ($2500) AND EMPLOYEE UNABLE TO PERFORM REGULAR WORK.
