While loading oversize rock into the haul truck a large rock jolted the truck causing the truck operator to sway side to side causing the operator to hit their head on the side door window. The loader operator saw the truck shake but didn`t know the truck operator hit their head.

While putting a 30' piece of channel frame conveyor into the shop to avoid the rain the straps on one end slid off the forks on the telehandler allowing the end of the conveyor to fall aprox 3' onto an employees left foot.

Attempiting to clean return roller when hand slipped and left ring finger was caught between return roller and conveyor belt.

Triaxle driver was being loaded with R8 size rock. When the rock was placed in the truck bed, driver was bumped out of his seat and into the passenger seat and felt back pop.

EE SAID 30 DAYS ATER HE HAD BACK PAIN THAT HE THOUGHT RUNNING HAUL TRUCK THAT THIS IS WHAT HURT HIS BACK, BUT THIS INCIDENT WAS NOT REPORTED TO ME UNTIL 30 - 40 DAYS LATER.

THE EMPLOYEE WAS WATERING THE ENTRANCE ROAD TO THE QUARRY USING A WATER TANK TRUCK. THE TRUCK WAS TRAVELING ON A DOWN HILL GRADE TOWARDS THE SCALES AND SCALEHOUSE. HE SWERVED INTO AN EARTHEN E MBANKMENT TO AVOID COLLIDING WITH A TRIAXLE TRUCK WHICH WAS BACKING UP OFF THE SCALES BECAUSE IT WAS OVERLOADED. THE ACCIDENT CAUSED THE TRUCK TO OVERTURN. HE WAS WEARING A SEATBELT WHICH WAS

VICTIM WAS CLEANING FROZEN ROADWAYS AROUND STOCK PILE AREAS WHEN CORNER OF LOADER BUCKET HOOKED A PILE CAUSING THE MACHINE TO STOP ABRUPTLY.

CHANGING WATER PUMP ROCK CAME OUT FROM UNDER THE LOADER TIRE HIT IN R KNEE BRUISING IT

SUPERVISOR AND CREW WERE BLDG A WALL AT THE MILL OPERATION. AT APPROX 1115 THE DAY OF 6-22-89 IT WAS TIME TO BACKFILL THE WALL. EE STOOD ON TOP OF THE WALL DIRECTING THE LOADER IN WHO WAS CARR YING THE BACKFILL. THE LOADER OPERATOR STARTED TO DUMP HIS BUCKET LOAD AND A LARGE ROCK ROLLED OUT OF THE BUCKET AND HIT THE TOP TIE HE WAS STANDING ON. HE FELL BETWEEN THE WALL AND THE EMBAN
