While the bus was loading with employess, a co-worker set in the seat behind me and shoved the seat forward; it was in the reclined position. When this happened I was laying on my right side; this immediately sent a sharp pain from my shoulder to my neck. Qnce we were at the office on mine site I notified someone. The pain is in my right shoulder, into my neck and down my arm.

Mostrando los 11Driller had back to equipment and was talking on the phone. Helper backed drill pickup up road, struck driller in back and knocked him down. Driller was unable to scramble out of way and right rear tires of truck ran over driller's legs between knee and ankle. Contusions to left leg - no broken bones.
Worker was driving loaded water truck down incline. Truck popped out of gear, and before it was fully stopped he went off the graded road onto rough terrain. Driver was bounced around.
WHEN HAULING A FULL LOAD OF WATER, EE OVER CORRECTED TURN CAUSING THE REAR WHEELS TO RIDE THE BERM AND FLIPPING THE TRUCK ON ITS SIDE CAUSING LOWER BACK PAIN
A 777 Ames haul truck collided with a 190T Cortez haul truck. After collision the Ames ee drove the 777 off the haul road to clear traffic. The track went down a slight incline causing torque to the cab. This popped the bottom of the windshield up inside the cab, striking the ee in the forehead.
EE POWERED THROUGH BUMP BERM. HAUL TRUCK WENT DOWN DUMP FACE AT ANGLE OF REPOSE, ABOUT 20'. REAR BRAKES WERE FULLY APPLIED. TRUCK FLIPPED UPSIDEDOWN, COMING TO REST WITH ALL FOUR WHEELS IN THE AIR. DRIVER HIT RIGHT SIDE OF HEAD, RESULTING IN A SKULL FRACTURE.
EE WAS SITTING IN TRUCK THAT WAS STRUCK BY A PORTION OF THE SHOVEL BUCKET CAUSING HIM TO BE JOLTED. RESULTING IN A MINOR NECK STRAIN. EE WAS SEEN BY A DR. AND PLACED ON LIGHT DUTY.
EE WAS OPERATING A SCRAPER STRIPPING TOPSOIL AT A GRAVEL PIT ALONG THE ROAD. THE PUSHER WAS COMING UP BEHIND THE SCRAPER TO ASSIST PICKING UP SOIL. THE EE SAYS HE WAS HIT VERY HARD. HE WAS TWI STED SIDEWAYS LOOKING BACK TOWARD THE DOZER WHEN THE DOZER MADE CONTACT, THROWING HIM FORWARD & HURTING HIS NECK, SHOULDER, & BACK.
WHILE DRIVING CAT 777C WATER TRUCK EMPLOYEE STATES THAT HE LOST HIS BRAKES. THE TRUCK WENT OVER ONE NERM AND ONE BENCH. EMPLOYEE RECEIVED CONTUSIONS AND LACERATIONS WHICH REQUIRED SUTURES.
ON 4-23-01 EE WAS OPERATING BOBCAT LOADER AND NOTED PAIN IN BACK. EE WAS SEEN BY PHYSICIAN ON 4-26-01 AND RELEASED TO FULL DUTY. PAIN IS ONTINUING ADN EE HAS BEEN PLACED ON LIGHT DUTY AS OF 5- 30-01.
EMPLOYEE WAS DRIVING A 240 TON HAUL TRUCK THAT WAS STRUCK ON THE RIGHT SIDE BY A 310 TON HAUL TRUCK WHICH HAD SKIDDED ON A WET ROAD SURFACE. EMPLOYEE WAS RELEASED BY PHYSICIAN TO WORK 2 DAYS LIGHT DUTY THEN RETURN TO FULL DUTY.
