THE EE WAS LEAVING THE TRUCK HE WA S IN WHEN THE FIRST TRUCK HIT THE FRONT OF HIS TRUCK. THIS CAUSED HIM TO FALL ONTO THE WHEEL OF THE LOADER THAT WAS PREPARING TO LOAD HIM.
EMPLOYEE WAS DRIVING A HAUL TRUCK HAULING FILL TO BUILD A ROAD INTO THE QUARRY TRUCK WAS BACKING DOWN A GRADE WHEN IT LOST ITS BRAKES. THE TRUCK ROLLED BACKWARDS INTO THE BANK CAUSING IT TO ROLL ON ITS SIDE.
