Employee was driving forklift 305 to EE's work area when the brakes failed. EE was unable to stop the forklift and came into contact with the barricade around a fire hydrant. As a result, EE experienced lower back pain and required medical attention.

Mostrando los 9Accident:Hooking bag straps onto forklift forks. When moving the forklift in place to lift a supersack bag the employee had his hand pinched between the forks and the straps of the supersack bag. This caused his first two fingers on his right hand to be cut. Employee was using all required PPE at the time of accident. ie-gloves.
Breaking down electric manlift at end of job. Employee was moving the lift backwards and stated he had the pipe in view. He released the pedal and the lift drifted and wedged him between the pipe and control panel.
The employee was reaching up to apply a hand brake to a railcar and felt a sharp pain in his shoulder as he tightened the hand brake. Note: This injury was not reported to site management until 08/05/2013.
While pushing material, the front end loader started bouncing and then I felt a sharp pain. It wasn't till an hour & a half later after I got home that my back starting hurting.
EE WAS "ROLLING" A RAILCAR TOGET IT IN THE CORRECT LOADING POSITION. THE BRAKES ON THE CAR FAILED. IN ORDER TO AVOID HITTING THE NEXT RAILCAR, THE EE JUMPED OFF INTO SOFT SAND, TWISTING HIS LE FT KNEE.
EE OBSERVED AN OBSTRUCTION INT HE DISCHARGE OF A 3 INCH HELICAL SCREW FEEDER. WITHT HE SCREW ENERGIZED, HE REMOVED THE EXPANDED METAL GUARD AND PLACED HIS HAND INTO THE FEEDER TO REMOVE THE OB STRUCTION. BOLTS ON THE SCREW PULLED HIS HAND BENEATH THE SCREW CAUSING LACERATIONS AND 2 FRACTURES.
EE WAS ADJUSTING (TRACKING) CONVEYOR BELT ON K-110 CONVEYOR IN THE BLUNGING AREA. THE ALL THREAD ADJUSTMENT WAS STRIPPED. THE BELT WAS RUNNING & THE GUARD WAS OFF. HANDS CAUGHT BETWEEN PRY BAR AND FRAME. HIS LEFT THUMB WAS MASHED AND HIS RIGHT INDEX FINGER WAS CUT.
