Operator stated that while the rock truck was being loaded, dust/debris had blown into the open window and into left eye causing irritation. The on site medic diagnosed that their cornea was scratched. Antibiotics were prescribed to prevent infection.

EE was lifting the cab of a bobcat skid steer. As the cab was lifted up, the door fell closed, catching the tip of EE's left middle finger. This resulted in a fracture to the tip of the finger requiring sutures.

Mud on transmission linkage prevented shifting from park. Driver exited the running pickup, set chocks, then went under pickup. While cleaning linkage, driver accidentally separated the shift cable from the transmission linkage. The operator unknowingly moved linkage from park to reverse. The wheel chocks were not set tight against tire allowing contact with driver at right flank.

Operator lost control of a fully loaded Caterpillar 777 F Model haul truck while attempting to make a left hand turn into a dump area. When applying the brakes ee lost traction and the equipment rolled over onto it's right side.

Grinding Operator reached through a gap between two guards to remove a plugged spray bar at the 2B conveyor. While attempting to remove the spray bar, it contacted the conveyor and wrapped around the bend pulley. The spray bar was ripped from the operator's hand resulting in a partial amputation of the operator's right ring finger tip.

While operating a loader, employee notified the Supervisor of excessive exhaust fumes in the cab. Mechanic discovered the loader hydraulic breather for the steering oil tank exceeded service life, and also found a small hole in an exhaust hose.

Operator of 777 haul truck ( #7714 ) was hauling from 925-332 shot to waste dump #5. When the operator was coming in for lunch felt a sharp pain in lower back and couldn't bend over or twist. The Supervisor asked if there was anything that happened that could of caused the back pain and the operator said not sure what happened.

The driver was heading from the Anarraaq met station site back to RDM when the snowmachine broke through thin ice atop overflow. The snowmobile began to zig-zag, he lost control of the machine and it tipped over. When the driver got up, he had pain in his right elbow and the passenger his right knee and forearm.

A 777 haul truck driver complained of neck soreness at the end of his shift. The operator was unable to attribute the soreness to any one event, however did not have soreness at the beginning of the shift.

A loader operator backing up with a bucket of ore and hit the back of the haul truck bed with the side of the loader bucket. When the loader hit the truck the driver was shaken / jarrred which caused the injury.

There was a missing vent on the A/C vents upon which the employee stuck his hand up into and struck the fan that was rotating. This caused a laceration on the left middle finger of the employee.

Employee had his thumb pinched on the hinged portion of a haul truck door.

Operator was delivering two connex's of sand to the sand filter tent with the 8233 tractor trailer and 40' high deck. He was going to stop, shifting from 2nd gear to reverse when he lost his brakes and could not get the truck stopped. He drove the truck to the bottom of the hill and rolled it over before inpacting the hill.

While loading a ore truck, a chunk of ore fell out of the bucket on the right side. The operator did not see the chunk and ran over it, severly jarring the machine, causing a fast side-to-side movement of the neck and head of operator, straining the neck from impact.

Employee was operating a Haul Truck when he drove over a pot hole in the road, which in turn had bottomed out the seat he was sitting on.

PT CUT HIS RT. HAND ON A CONVEYOR BELT WHILE WORKING AT THE PORT THIS EVENING.

HURT HIS LOWER BACK LAST NIGHT WHEN HIS SEAT BOTTOMED OUT IN HIS VEHICLE.

EE WAS OPERATING FRONT END LOADER. DUE TO THE ROUGH RIDE OF THE LOADER, THE EE FELT A TIGHTNESS IN HIS LOWER BACK.

EE WAS REMOVING A PIECE OF LUMBER FROM THE SCRAP BIN INSIDE THE MILL AND WAS SQUEEZED BETWEEN A ROLL OF BELTING BEING MOVED BY A FORKLIFT AND A STATIONARY SCRAP BIN.

WHILE ATTACHING THE CABLE TO THE REAR HITCH OF THE WALDON, THE WALDON DRIVER BACKED UP, BRIEFLY PINNING EMPLOYEE BETWEEN THE MACHINE AND THE I BEAM OF THE CSB.

EMPLOYEE WAS ASSISTING TO CHANGE OUT A 10X10 PUMP CASING. AS THE CASING WAS BEING LIFTED OVERHEAD THE SLING BROKE AND THE CASING FELL APPROX.10' TO THE FLOOR. IT STRUCK A SPOOL PIECE AND DEFLECTED AND STRUCK AND PINNED EMPLOYEE. CASINGWEIGHS 1700LBS.

EE WAS USING A SNOWMACHINE WHILE COLLECTING WATER SAMPLES THE MACHINE BECAME STUCK & HE HURTHIS BACK WHILE TRYING TO GET IT UNSTUCK

EMPLOYEE JUST STARTED TO WALK AWAY FROM THE CONTROL ROOM, WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK. HE HAD BEEN USING A WASHDOWN HOSE EARLIER.
