He opened up the guard and stuck his Right hand in, past bearing for the tail pulley, with a pair of Channel locks to clean. When he lost his footing, slipped and fell into the belt, the tail pulley grabbed his channel locks and pulled him into the tail pulley.
Haul Truck driver lost his brakes coming down a grade. His left rear duals caught fire. He steered the truck into the berm and went off the road and tipped his truck over on to its right side. The driver was injured and taken to the ER room and was released about 4 hours later.

EE was operating TRK #86 at the smith mine and on his way out of the mine in the main when he hit some pot holes causing him to strike the top of the cab with his hand hat.
