EE was opertating a MT436 and felt pain in low back when driving.

Employee-operated Kubota was struck by 40T haul truck operated by mine owner at a travel way intersection of the underground mine. The employee suffered fractures in employee's legs and was transferred to a hospital for treatment.

EE was holding onto the top rail of manbasket on a kubota tractor when the gate swung fully open pinching right ring finger in the gap resulting in a fracture of the distal phalange.

Employee was testing and troubleshooting an air conditioner condenser fan that was not functioning and amputated the tip of left pinky finger.

EE reports back strain while working as a loader / haul truck operator.

On 10/07/2015, employee complained of strain/kink in right side of neck and shoulder while driving a piece of mobile equipment.

Employee missed a gear causing equipment to contact rib. Employee struck head on window.

Employee was driving up an incline and met another vehicle coming down the same incline. Employee stopped the tractor and attempted to reverse to back up. The tractor rolled back during the attempt and came to a stop on the rib. On 3/25/2014, employee was evaluated by a physician and given time off for a cervical neck strain.

Employee reports that he was traveling down an incline and something made his vehicle bounce. When the vehicle bounced, the employee's head made contact with the roof of the cab. He continued to work his entire shift without incident. He woke up several hours later while off shift and felt pain in his neck.

Employee was driving a haul truck on surface ore pad. After dumping his load, he drove down from the ore pad when he encountered some ruts in the road. The ruts caused the employee to hit his head on the right side of the window inside the cab. This resulted in a concussion.

Employee was in a loader/mucker carrying a load rock. As he was moving with a loaded bucket, he ran over a bump in the road causing him to hit his head on the top of the cab of the mucker. Initial medical evaluation cleared the employee without restrictions. Employee notified employer on 02/07/2012 that he saw his physician for continued symptoms resulting in lost time.

Employee had been injured while working around the house on days off, condition was aggravated by work. Symptoms include numbness and tingling to left lower extremity. Symptoms increase while sitting for short periods of time. Employee referred to his personal physician for evaluation.

Employee with history of back injury & pain was driving truck when experienced pain in back. He did not want any treatment & went to his own Dr & returned to work on his next scheduled shift. He did not tell the company that he had been to the Dr. His Dr notified the company that he was unfit to work. Employee was removed from duty & sent home until cleared by his Dr.

Truck hit a bump and elbow hit the estop. Employee sought medical treatment and was released to work. A fracture was not suspected at the time of incident. The employee reported back to the medic after a week of long change and reported swelling and stiffness of finger. It was found that the force of the truck stopping resulted in a fracture to his left ring finger

Truck driver was hauling pastefill to stope. He completed 16 loads and during the course of the day his back became sore. He reported the incident to his supervisor and went home. He woke the next day with back pain and went to see his Doctor. There was no single event that could be attributed to the injury.

While riding in back of tractor, after exiting portal tractor hit bump and bounced, causing employee's loose cap lamp to come off hardhat, swung around on cord and strike him in the teeth, chipping tip of incisor tooth and requiring cap.

Employee was directing boom truck operator of where to move a load of pipe. The load moved too far pinching employees little finger against another piece of mobile equipment.

Operator was back dragging with the loader bucket, placed his face near the frame of the loader to get a better view. At the same time, the bucket cylinder knuckle connection overcentered, jarring the operator. He made contact with the loader with his safety glasses pushing them against the bones in his face causing injury.

Employee was mucking out a round with a loader. The bucket became lodged on a rock or an undercut on the face. The employee rolled the bucket back causing the rear end to raise up. The bucket suddenly slipped off the face and the release of mechanical energy caused the loader to slam down hard on the ground sending a shock through the machine to the operator

EMPLOYEE WAS ASSISTING IN MOVING AN I-BEAM WITH ANOTHER EMPLOYEE OPERATING A FORKLIFT. AS THE FORKLIFT LIFTED THE BEAM, THE BEAM MOVED AND CAUGHT THE EMPLOYEE'S FOOT BETWEEN THE BEAM AND SOM E 4 X 4 DUNNAGE. EMPLOYEE'S METATARSAL OF RIGHT FOOT WAS BRUISED.

MINER WAS SCRAPPING RIB WITH LOADER BUCKET. AN UNEXPECTED PIECE OF ORE FELL FROM RIB RIGHT BESIDE OP COMPARTMENT. A 100 LB PIECE OF THE ROCK BOUNCED OFF OF THE EQUIP ROPS AND STRUCK OPERATOR O N THE BACK.

WORKER WAS RIDING IN TRACTOR MAN CARRIER BASKET THE TRACTOR OPERATOR DID NOT SEE A RIDGE OF SNOWLEFT BY PLOW. THE TRACTOR HIT & JUMPED OVER SNOWPILE CAUSING WORKER TO STRIKE LOWER BACK BODY AG AINST MAN CARRIER GUARD RAIL

OPERATOR WAS DRIVING MINE TRACTOR DOWN RAMP. VEHICLE HIT A POTHOLE IN ROADWAY AND OPERATOR FELT SHARP PAINS IN HIS NECK.

WHILE SLINGING A LOAD WITH THE FORKLIFT, THE CHAIN SLIPPED OFF CAUSING THE LOAD TO FALL TOWARDS HIM HITTING HIM IN THE FACE.

EE WAS RIDING ON THE BACK OF A TRACTOR AS THEY STARTED OUTSIDE A SLOWER TRACTOR PULLED OVER TO ALLOW THIN TO PASS. THE TRACTOR STRUCK A ROCK IN THE DRIFT CAUSING THE TRACTOR TO BOUNCED AT THIS POINT EE STRUCK HEAD ON R.O.P RESULTING IN HARDHAT HITTING BRIDGE OF NOSE CAUSING CUT AND LATER 12 STITCHES.

EMPLOYEE WSA OPERATING A LOAD HAUL DUMP (LHHDH) UNDERGROUND WHEN EUQIPMENT HE WAS OPERATING RAN OVER A ROCK IN THE HAULAGE WAY CAUSING THE LHD TO BOUNCE UP AND DOWN MORE THAN NORMAL, RESULTING IN EE TO HIT HIS HEAD ON THE R.O.P.S. CAUSING A LACERATION TO THE FOREHEAD.

EE BACKED UP CLOSE TO THE EDGE OF THE ROADWAY ONTHE DRIVER'S SIDE. HE STOPPED & PULLED FORWARD, TURNING THE TRUCK AWAY FROM THE EDGE. WHEN HE PULLED FORWARD, THE ARTICULATION INCREASED THE TRA VEL RADIUS OF THE BACK TIRES & THE WHEELS LEFT THE ROADWAY. THE TRUCK ROLLED INTO THE POND.

EMPLOYEE WAS A PASSENG4ER ON A EXPLOSIVE VEHICLE (PT -21) & WHEN THE VHICLE CAME TO A SUDDEN S TOP THE EMPLOYEE CLAIMS THAT HS BODY BENT BACKWARDS HE CLAIMS T HAT WHILE BENDING HIS NECK HE HE ARD A POP & FELT T HAT HIS NECK WAS OUT OF PLACET HE EMPLOYEE WAS EXAMINED BY A CHIROPRA TOC & TREATED WI TH CHIROPRACTIC MANLPULA TION ( REPORTABLE ONLY)

THE EE WAS A PASSENGER ON A MINE TRACTOR THAT HAD A SEAT AND SECRTBELT PROVICED FOR A PASSENGER THE OPERATOR TURN LEFT AND THE EE CLAIMS THE SEAT BELT BROKE AND ALLOWED HIM TO FALL TO THE GROU ND STRIKING HIS RIGHT KNEE. THE EE WAS EXAMINATEPHUYSICIAN AND DIGNOSED AS HAVING A CONTUSION OF THE RT KNEE.

54736YEE TOLD HIS SUPERVISOR 08-13-92 THAT HIS BACK WAS SORE FROM OPERATING A HAUL TRUCK.THE EMPLOYEE WORKED UNTIL HIS SCHEDULED DAYS OFF 08-15-92 AND RETURNED 08-16-92 WITHOUT COMPLAINING OF 15807AIN.WITH OUT AUTHORIZATION FOR TREATMENT THE EMPLOYEE SCHEDULED AN APPOINTMENT WITH A CHIROPRACTOR 08-17-92 AND 08-18-92.THE EMPLOYEE RETURNED TO WORK 08-19-92 STATING HE HAD BEEN UNDER T

EMPL WAS HAND MUCKING FACE AS SCOOPTRAM CAME TO BACKDRAG. EMPL WAS BENT OVER & SCOOPTRAM OPERATOR DID NOT SEE EMPL AT FACE. SCOOPTRAM BUCKET HIT EMPL IN LEFT LEG BELOW KNEE & RESULTED IN OPEN FRACTURE. LEG WAS LATER AMPUTATED JUST BELOW KNEE.
