EE ARRIVED AT BENCH AREA AND PARKED THE KUBOTA, SET THE PARK BRAKE AND WALKED DOWN TO BENC TO TIE IN ROUND. THE PARK BRAKE FAILED TO HOLD AND THE KUBOTA ROLLED DOWN, STRIKING EE ON HIS LEFT CA LF. REQUIRED STITCHES.
EE WAS PULLING PIPE OUT OF A 9' X 9' DRIFT WITH A LOADER. THE PIPE HIT A ROCK, WENT UP & SWUNG TO THE SIDE, HITTING WORKER ON THE LEG.
WORKERS WERE USING SLURRY TRUCK TO RECONNECT PIPE. THE LEFT-TO-RIGHT CONTROL ARM ON BASKET BECAME STUCK AND SWUNG TO THE LEFT SIDE OF EQUIPMENT. THE VIC PIPE SNAPPED ON BASKET AND WITH PRESSUR E, FREED ITSELF, STRIKING HIM ON THE HIP AND BACK.
EE WAS DRIVING HAUL TRUCK TO BE SERVICED, TRUCK STALLED, TRIED TO ENGAGE PARK BRAKE & WOULD NOT ENGAGE. HELD TRUCK WITH SERVICE BRAKE PEDAL. MECHANICS ARRIVED TO CHECK BRAKES & PRESSURE ON SER VICE BRAKE RELEASED, SENDING HAUL TRUCK BACKWARDS ON DECLINE, TOSSING EE BACK & FORTH IN CAB.
EE & COWORKER WERE COMING UP THE DECLINE, THEY SAW THE #908 DUMP TRUCK STALLED, HE CLIMED INTO THE TRANSMISSION COMPARTMENT TO CHECK ELECTRICAL SYSTEM. OPERATOR OF TRUCK ALERTED HIM TRUCK WAS ROLLING BACKWARDS. HE STAYED IN BACK OF DUMP TRUCK & SUSTAINED BRUSISES TO RIGHT ELBOW & LEFT BUTTOCKS.
EE & COWORKER WERE COMING UP THE DECLINE, THEY SAW THE #908 HAUL TRUCK STALLED. HE CLIMBED ON ENGINE COVER & OPERATOR OF TRUCK ALERTED HIM THE TRUCK WAS ROLLING BACKWARDS. EE WAS THROWN OFF TR UCK. HE SUSTAINED A FRACTURED LEFT ELBOW & SPRAINED RIGHT ANKLE.
THE ORE CAR EMPLOYEE WAS RIDING IN DERAILED BOUNCING HIM AROUND IN THE CAR.
EMPLLOYEE WAS GOING TO SERVICE EQUIPMENT. HE WAS DRIVING ON A WET SURFACE AND HAD TO APPLY THE BRAKES. HE SLID SIDEWAYS INTO RIB AND FELL FORWARD, HITTING HIS RIGHT SIDE.
EE WAS LOADING FACE ROUND AND FINISHED LOADING TOP ROW. HE INTENDED TO LOWER BUCKET, BUT PUSHER CONTROL LEVER WRONG WAY AND SLAMMED INTO THE BACVK, CATCHING HIS RIGHT HAND BETWEEN THE BACK AND THE LEVERS.
EMPLOYEE WAS JUMP-STARTING A POWDER LOADER, HE WENT TO TAKE TH JUMPER CABLE OFF KUBOTA WHILE THE ENGINE WAS RUNNING AND CAUGHT HIS RIGHT MIDDLE FINGER KNUCKLE IN FAN.
WHILE DRIVING THE SECURITY TRUCK [UNIT #25] THROUGH THE CREEK ON MILL. #1 ROAD AT 15-20 MPH, VEHICLE WHEELS [LEFT FRONT & REAR] STRUCK BUMP IN ROAD. EE STRUCK HEAD WITH HARD HAT ON ROOF OF VEH
AFTER UNLOADING ONE LOAD [ONE DRUM] THE EE WAS RETURNING TO PICK UP ANOTHER OIL DRUM. THE FORKS WERE IN 1/2 HIGH POSITION AS HE APPROACHED THE BARREL AREA. EE STARTED LOWERING THE BOOM. EE'S F OOT WAS CAUGHT BETWEEN THE BOOM & THE FRAME OF THE MUSTANG. [EE HAS HIS RIGHT FOOT ON THE BOOM FRAME.
THE 931 POWDER TRUCK WAS IN THE SHOP TO BE WORKED ON. THE EE WAS TESTING THE BASKET SWING WHEN HE HIT THE LEVER. THE BOOM SWUNG OVER AND KNOCKED EE OFF THE TRUCK. HE LANDED ON HIS RT WRIST.
EMPLOYEE WAS RIDING ON BACKOF TRAIN. CAR WENT UDNER CHUTE A BOULDER HIT THE CHUTE AND CAME BACK AGAINST HIS HAND WHICH WAS HOLDING ONTO THE BACK OF HTE ORE CAR.
INJURED PARKED EUCLID TRUCK, WAITING TO BE LOADED. LOADER OPERATOR FAILED TO OBSERVE TRUCK & BACKED LOADER INTO TRUCK. TRUCK DRIVER SHOULD NOT HAVE PARKED UNTIL LOADER OPERATOR INDICATED WHERE HE WANTED THE TRUCK PARKED.
EMPLOYEE WAS INVOLVED IN ORE HAULAGE HE WAS RIDING ON BACK END OF AN ORE CAR THAT HAD BEEN OVERLOADED A LARGE BOULDER STRUCK HIM IN THE CHEST AS A RESULT HE WAS KNOCKED OFF THE CAR & FELL ON HIS BACK TO THE BOTTOM OF THE DRIFT BOULDER STRUCK HIM AS HE LAID ON THE BOTTOM THE DRIFT CAUSING THE INJURY TO HIS LEGS
EE WAS OPERATING AN ST-8 SCOOPTRAM. HE RAN OVER LOOSE ORE ON THE FLOOR OF THE DRIFT, CAUSING CANOPY OVER THE OPERATOR'S SEAT TO STRIKE THE BACK. THE FORCE OF STRIKING THE BACK WAS SUCH THAT TH E METAL POST THAT SUPPORTS THE CANOPY BROKE AND CAUGHT HIS FINGER BETWEEN THE POST AND THE CONTROL LEVER, CAUSING INJURY TO HIS LEFT MIDDLE FINGER.
EE WAS OPERATING THE SCOOPTRAM ST-8 WHEN THE BUCKET LIFT HYDRAULIC HOSE CAME LOOSE FROM THE HYDRAULIC CYLINDER. THE WEIGHT OF THE ORE IN THE BUCKET CAUSED THE HYDRAULIC OIL, CONTAINED IN THE C YLINDER TO BE SPRAYED ON HIS UPPER BODY HANDS ARMS, FACE AND EYES THE TEMPERATURE OF THE OIL WAS QUITE HIGH, APPROX. 180 DEGREES CAUSING BURN TO BODY PARTS LISTED.
