A driver was exiting mine site and while exiting the mine lost control of the truck due to traveling too fast down a steep grade. The driver tipped the truck and trailer over while attempting to navigate a tight turn. The driver side window was down and the drivers left arm went out of the window when the truck tipped. EE's left arm had to be amputated just below the elbow.

The EE was training in a mine haulage truck sitting in the buddy seat.

Was sitting in haul truck watching shovel and felt a pop in back. There was no MOI

Employee was operating motor grader, other employee was approaching in haul truck, haul truck operator hit brake and started to skid, when mg operator turned. Haul truck operator hit mg.

employee was standing at the rear of pickup when another employee backed pickup and ran over both legs

Employee was in T-75 and was under shovel getting loaded. A rock came off shovel and hit headache rack. Employee complained about neck pain.

EMPLOYEE WAS TURNING WATER TRUCK AROUND ON HAUL ROAD, ENGINE DIED & TRUCK LOST ALL POWER. TRUCK WENT ON DOWN GRADE & HIT BERM CAUSING THE TRUCK TO ROLL OVER ON HAUL ROAD.

THE EMPLOYEE STATED THAT HE WAS TRAVELING ONT EH MINESITE ACCESS ROAD TO THE DESIGNATED PARKING ARE FOR THE EMPLOYEES. THE EMPLOYEE STATED THAT HE WAS IN HIS PERSONAL VEHICLE TRAVELING ON TEH ACCESS ROAD BY DRIVE HOUSE #1 WHEN A LEDCAR (CONTRACTOR) PERSONAL VEHICLE CAME UP THE ROAD ON THE WRONG SIDE HITTING EE HEAD ON. EMPLOYEE IS GOING IN FOR SURGERY ON HIS LEFT SHOULDER.

EMPLOYEE STATED THAT HE WAS STANDING ON THE WRONG SIDE OF THE POWER CABLE THAT THE SUPERVISOR WAS PULLING WITH HIS PICKUP. HE STATED THAT THE CABLE HIT HIM KNKOCKING HIM TO THE GROUND, AND WH EN HE FELL, HE STRUCK HIS SHOULDER ON THE HARD ROCK GROUND.
