Mostrando los 26Moving a charging carousel for batteries, ee was using a forklift to lift a hinged structure known as a wing plate. A small piece of metal had become lodged in the wing plate's hinge. When the plate was lifted, metal was subjected to a significant amount of force which caused it to be projected toward the employee at high velocity, striking the ee in the cheek.
Employee was traveling underground in a mantrip checking ventilation line on the main travel way. The vehicles passenger side front fender came in contact with pillar of mine causing it to come to a sudden stop. As a result employee taken to hospital.
Emp #1 was operating a ramcar & was parked behind a miner. Ramcar lights were off while ore was being loaded. Emp. #2 was driving another ramcar in reverse, approaching the miner with trailer lights off. Emp #2 didn't see Emp #1 being loaded behind the miner & bumped Emp #1 ramcar. Emp taken to Er per request. Prescription given for pain. Emp. back to work with no restrictions.
Two emp. were operating the suction line off the vacuum truck. One employee got left arm pulled into the suction hose. Second emp. shut the truck off. Emp. taken to ER and given a prescription pain med.
Employee was performing clean-up duties using a Belt Tractor. The employee was in the process of backing up when his left foot was caught between the tractor and rock ledge. The employee's foot was outside of vehicle at this time. Emp. taken to ER sutures and pain med given. Small fx to small toe left foot.Restrictions given.
EE just finished washing out the bed of the ram car. As EE was stepping up into the cab he lifted his left leg and put his leg in the cab then as he lifted his right leg EE felt a pop in his right knee. EE taken to ER per request. EE placed on restrictions and given a pain injection.
Emp. was driving up the slope underground, he fell asleep and drove under the conveyor belt. Emp. was taken to the ER and received 6 total stitches and placed on restrictions until 6/11/12.
Emp. riding in a mantrip as a passenger. Driver of mantrip hit the side of the rib. Vehicle turned slightly sideways and emp. hard hat and safety glasses flew off his head. Emp. denied need for medical attention; however the following day he had left shoulder, neck and back pain. Emp. taken to the dr. and placed on restrictions.
Emp. was driving CU106 Boom Rig around a rib at main shop. He hit a pothole which caused the air ride seat to hit bottom and jar his lower back. He said he felt no immediate pain. Later in the day the pain increased. When he went home for the weekend he started taking Oxycontin. Injury reported on 1/10/11.
Emp. was unloading air boat off trailer into the water. To get the boat to start turning away from the bank emp. had to push the boat using the back of her legs to get the boat to start turning. Emp. felt the back of her right knee strain.Emp.taken to Work Comp. doctor and restrictions given on 11/23/10.
Emp. was using a large forklift to move a motor. He was attempting to adjust the forks to pick the motor. The forks did not move freely, he then tried to push one fork over with his left foot, at that time he felt his knee pop. He grabbed the mast to keep from falling.
Emp. drove shuttle wagon to get 3 cars. Emp. hooked up 3 rail cars to transport to loadout station. Emp. was slowing down approaching the previously staged cars when the stray car hit the back of the cars that were attached to the shuttle wagon. The force from the collision caused the emp. to slam into the steering wheel. Emp. taken to ER for evaluation.
A loader opr was trying to loosen a guard rail pipe by using a loader to lift the rail. The rail pipe broke due to the force being applied and flew toward emp. The pipe hit emp's hard hat and knocked him to the ground. Emp had laceration from lining of hard hat and was sent to doctor for eval. 3 staples were applied to laceration.
Employee was operating a scoop on a slope underground when the engine on the scoop died and it started to roll down the slope. The emp fell out of the scoop and landed on his rt leg resulting in a fracture. Went to ER, received surgery, was hospitalized and lost time from work.
Emp was driving a tractor, dragging a skid for a prolonged period of time. The crew was behind him unloading the cable off of the skid, so emp was constantly twisting his body to look over his rt shoulder. When he got out of the tractor, he felt a pain in the rt knee. The next morning the pain was increased. Went to ER due to company doctor closed. Rcvd restrictive duty.
Employee stated that he was traveling down the belt on a belt tractor, just made a left turn around the tail piece when he ran into hanging belt structure causing a cut to L upper arm and bruising to L forearm. Emp. to ER received 100 staples to L arm wound (surgical procedure) and a drain. Spent one night in hospital and placed on restricted duty as of 6-15-07.
Employee was driving in a low height area and bumped his head against the back. Initial X-ray was negative. Follow up CAT scan was negative. Doctor gave a prescription for muscle relaxant and prescribed one day off work.
NOTE: Unaware this had to be reported, as accident occurred in parking lot outside normal work activities and shift. Emp was parking motorcycle in company parking lot when the motorcycle fell on her, causing fracture to right knee which required surgical repair.
Emp had parked ramcar by the belt feeder to help with some cleanup at the end of his shift. He backed the car up and started down BT 41 toward the shop. When he reached down to turn on the lights, he stated that he hit the corner of the rib. The car was not traveling fast and there was no damage to the ramcar.
EE WAS OPERATING A SCOOP THAT GOT STUCK. TRYING TO FREE IT, HE PLACED A ROCK UNDER THE REAR TIRE AS THE TIRE WAS STARTING TO TURN. EE'S HAND WAS DRAWN IN BETWEEN THE ROCK AND THE TIRE. EE PULLED HIS HAND OUT OF THE PINCH POINT.
HOOKED ALL AIR TO CARS & STARTED DOWN HILL WITH SHUTTLE WAGON UNDER CONTROL. LOST AIR PRESSURE & CONTROL OF RAILCARS. EE JUMPED OFF SHUTTLE WAGON AND LANDED ON BOTH FEET.
EE WAS DRIVING 10% SLOPE AND APPARENTLY LOST CONTROL OF THE VEHICLE AT THE BOTTOM OF SLOPE. THE VEHICLE DROVE THROUGH A PLYWOOD BULKHEAD AND CAME TO REST WHEN IT STRUCK A RIB.
WHILE DRAGGING THE ROADWAY IN AREA 21 OFFSIDE TRAILER TIRE OF685 RAMCAR HIT CORNER OF RIB R73 BI36 CAUSING A ROCK TO STRIKE EE ABOVE LEFT EYE CAUSING A LACERATION APPROX 31/2" LENGTH. THE ROCK HIT THE TOP FRAME OF GLASSES.
EE WAS STRUCK BY A PIECE OF SLAB WHILE OPERATING SCOOP TO MUCK OUT THE BT BETWEEN BELT ROOM & ESCAPER IN R-683.
INITIAL INVESTIGATION LEADS US TO CONCLUDE THAT A PIN CAME OUT OF THE STERRING CYCLINDER ON #334 MANTRIP AT BT 7 ON THE 840 NORTH ROAD. THIS CAUSED EMPLOYEE TO LOSE STEERING CONTROL OF THE VEH ICLE AND IT VEERED TO THE LEFT STRIKING THE OLD ABANDONDED BELT LINE. THIS RESULTED IN EMPLOYEE RECEIVING LACERATION TO L. FOREARM THAT REQUIRED SURGICAL REPAIR.
