While operating a Club Car the employee reached out to push some vent line out of the way. In the process they received a laceration to the forearm. The laceration required 5 sutures to close. Employee was issued a full release.

Miner was assisting partner in loading a pallet with a power cable on it. While guiding the 2yd operator up to the pallet, the bucket of the loader contacted the Miner's ankle, resulting in a fracture.

The employee reports was traveling down the 75 fwl in a haul truck on good roads. The parking brake set up on the truck while in motion causing the employee to twist lower back. The employee was wearing seat belt at the time. The employee was seen by a doctor and issued restrictions.

Employee was working out of a 2 yard UG loader bucket installing pipe when EE's partner lower the bucket EE's left hand was resting on the back of the bucket and third and four fingers were broken when they were pinched between the bucket and the Z Link of the lift cylinder.

Employee reported EE was sitting in a mantrip, the train jerked forward and stopped unexpectedly causing neck pain. Employee was seen by site medic and referred to Occ Health. On 7/3/2024 EE was issued light duty restrictions.

Employee was mucking with 2-yard loader on last shift 5/24/24 when employee strained upper back muscles. Seen medic & was released. After days off employee stated their back was still hurting, taken to ER on 5/28 & was scheduled to see an occupational health physician on 6/6/24 restrictions were issued at this time making incident reportable on 6/6/2024

Employee was attempting to pull up stabilizer jacks on lift truck when lift truck failed mechanically resulting in the lift bed to raise on its own. The employee's left wrist/hand was pinned between the controls & vent tubing for an extended time. Leadman miner found employee & got employee down. X-rays were positive for fracture to left wrist.

Employee was loading haul trucks underground & upon dumping a scoop of muck EE proceeded to back around the pillar to get more muck when EE drove some oversize spillage resulting in the loader to bounce jarring EE's low back. Seen site medic, referred to ER, then onto Occupational Health specialist where restrictions were issued for muscle strain.

Employee riding in mantrip vehicle when it hit a bump causing the employee to be jarred, resulting in lower back pain on left side of back. Seen by doctor and given a full work release. On 5/25/23 employee was seen again due to persistent pain at which time, restrictions were issued limiting the ability to perform all required job duties.

While riding out of the mine on 5/21/23 employee stated was jarred in the seat resulting in back pain. EE did not report this to management. On 5/24/23 while on days off EE reported to the supervisor that EE was seeing a doctor for continued back pain resulting from the incident on 5/21/23. Restrictions were issued limiting EE from performing all required job duties.

Employee was riding in a light vehicle coming out of the mine when a foreign body entered left eye. Employee washed it and saw site medic, was referred to an eye specialist, they were diagnosed with a rust ring starting and had it removed and was given restrictions for 3 days.

Utilizing a forklift around rail flat, one fork was inadvertently under a 1" lip of the flat. The uninjured employee applied lift to the forklift at the same time the injured employee reached across the forks. The fork under the lip slipped off springing upwards, contacting the employee in the chin causing a laceration and a broken mandible.

The Company was made aware on 9/11/22 when an employee reported off with work restrictions & stated on employee's last scheduled shift on 9/6/22 employee strained low back while operating a 2yrd loader. Pain worsened on days off so employee went to chiropractor and was issued work restrictions making this reportable.

The employee was traveling to work area in a man carrier. They attempted to activate a block light pull bottle while the vehicle was still in motion. their right hand came in contact with the roof of the vehicle, causing a laceration to the top of their right thumb. Employee was taken to an onsite Medic who cleaned and dressed the wound.

Employee was cleaning EE's roadway using a 2 yard loader, in preparation to muck, as EE was backing up to make another pass & ran over a rock EE missed on EE's 1st pass resulting in low back pain. Seen Site Medic, Referred to ER, Restrictions were issued.

While operating a underground cabbed haul truck employee hit a bump in the road causing them to bounce in the seat resulting in their head contacting the top of the cab liner resulting in a laceration to top of scalp. Sutures were required, full work release issued making this a medical reportable incident.

Employee was riding in the passenger seat of cabbed supply/delivery truck the operator hit a bump on roadway causing employee to move forward in seat, contacting a corner of instrument panel in cab with EE's left cheek/mouth area resulting in a puncture, two sutures were required, full work release issued making this a medical reportable.

While riding in the back of a covered man-trip employee stated that they hit a bump in the road and it jarred employee's low back. On 4/12/22 employee took a vacation day and went to primary care doctor and was issued restrictions for a low back muscle strain.

A mine supervisor was operating a UTV down an internal ramp, when EE stated the UTV began gaining speed and contacted the rib resulting in injury to the supervisor.

Employee reported that after making a sand line change & working on a pump they were traveling in an enclosed cab vehicle to next job when they felt something enter right eye. Seen site medic, referred to ER, where foreign body was found & removed, Referred to Optometrist who issued a preventative antibiotic and took them off work for 1 shift.

The employees were removing a steel plate attached (welded) to the floor using a cutting torch and a fork lift. The fork lift was under the plate and applying lift. The other employee utilized a cutting torch to cut the final weld and when the weld was cut the plate "sprung" up and landed on EE's foot causing two fractures in EE's left foot.

Employee's finger was pinched between the door of a loci and the rib. After EE called for the train to move EE reached up to close the loci door and finger was pinched against rib. EE was seen at site medic and eventually transported to a clinic where EE had surgery to treat injuries to two middle fingers.

Employee was removing 36 inch fiberglass vent tube from a heading utilizing a lift truck. Employee had one 10 section on the bed and it rolled about 2 feet pinching employee's right leg between the section and the side rails of the lift truck. Was seen by site medic, referred to ER, where restrictions were issued for muscle bruising.

Employee was standing near the face while their partner was on a 2yd loader preparing to position the loader bucket for loading. Employee gave partner direction to lift the bucket and in doing so the bucket lip caught a pvc collar pipe causing it to shatter, a piece flew up and struck employee in the left eye causing a laceration.

Employee was operating a 4 yrd, side loading own truck. After dumping a bucket into the the truck the mucker hit a pothole\rock causing the seat to compress and spring up. The operator contacted head on the cab of the mucker. Operator was wearing seat belt. Employee was transported to Livingston hospital and diagnosed with a cervical strain. Taken off work for 2 days.

Employee was riding in the back of a man carrier when the transport hit a bump causing a jarring motion. This motion caused a back strain to the employee.

Employee was operating a underground loader when EE drove over a pothole causing EE to jar neck causing pain and discomfort. Seen by site medic referred to ER & was released with instructions to seek further care if pain did not subside. Went to a physician the following day as pain was still present & was restricted at that time.

Employee & partner were hanging electrical cable from an lift truck when the controller used to lift the bed made contact with the fiberglass vent tube causing the bed to lift & pinching employee between the top rail and vent tube causing them to pass out. Seen site medic, referred to ER, x-rays negative, restrictions were issued.

Employee was operating a haul truck down a haulage ramp with another truck ahead of EE. As EE neared chutes EE was pulling from, EE saw other truck backing into the chutes so EE shut off the drive lights & went to brake but missed & bumped the throttle causing EE's truck to contact the bed of the other truck & then the rib. Doctor restricted EE for 1 shift for wrist sprain.

Employee was pulling a 6 inch drisco line into heading using a 2 yd. loader. As EE came around a corner & straightened out the drisco line contacted EE's back pushing EE forward causing EE's mouth to contact the loader. 2 sutures were required, x-rays positive for a chipped tooth, a full release was issued making this a medical reportable

Employee stopped the loader EE was operating on a ramp to talk to another employee. EE had set the park brake & shut the loader off to talk. Shortly after, the loader's park brake failed to hold & loader started rolling on its own down the ramp, causing injury to the operator before coming to a stop.

Tire tractor pulled into a cross cut to allow tomcar to pass by. While attempting to back further out of the way the tire tractor moved forward contacting the tomcar operator as EE drove past. The operator received a laceration to the arm and was seen by the on-site medic. EE was transported to St.Vincent Hospital and diagnosed with forearm laceration.

Employee was operating a 4 yd loader, while mucking out a borehole the loader bucket contacted the rib causing the loader to come to an abrupt stop causing the employee to contact chin on the left shoulder pad resulting in a laceration. Seen by site medic, referred to ER, two sutures required, no restrictions issued making this a medical reportable.

EE was pulling in to the 79 LOM with a loaded truck and struck a rock with front right tire causing EE's head to contact the roof of the cab. EE was transported to the hospital and diagnosed with a concussion and neck strain.

On 4/14/18 ee went to get into a running 4 yard loader and knee hit the controls causing the loader to articulate. EE was pinched in the articulation resulting in a hip injury.

Employee was backing up loader from the muck pile and the left rear tire of the loader stood up a blasted rebar bolt which contacted the employee on the back side of right leg near the knee causing a contusion. Seen by site medic, referred to ER were restrictions were issued.

Employee was mucking when hardhat contacted a piece of ground support. Employee was seen by the medic & sent to ER. Head & neck scans were negative. Restrictions were issued by doctor.

On 6/29/17, while driving a UTV an employee's gloved hand was pinched between the roll cage & the rib, resulting in a positive x-ray for the tip of finger & a laceration which required sutures. Employee was issued a full release. No days Lost.

On 6/28/2017 employee reported to the supervisor that EE had hurt neck while operating a 4 yard loader on 6/27/2017. EE stated that EE hit a bump in the road and jarred EE's neck but thought EE would be okay. EE was seen by site medic, referred to ER, X-Rays were negative, restrictions were issued for a strain.

Operating a powder truck employee had to negotiate a corner before descending a ramp EE switched to hand controls so EE could stand and see EE's load in the process of switching back to foot controls & sitting back down the vehicle came to an abrupt stop employee contacted the windshield with head & upper torso diagnosed with a concussion/restrictions issued.

Employee was at the front of a haul truck talking to the driver on a foot wall lateral that was stopped, engine off, & park brake set when a loader bumped the rear of the truck causing truck to move slightly forward making the front bumper contact the employees left knee. Seen by site medic, referred to ER where restrictions were issued.

On 1/26/2016 employee was operating a forklift and strained neck. Employee was seen by site medic & ER, full release was issued no days lost. On 3/7/16 employee was seen by doctor, who then issued restrictions, 3/11/16 is the 1st shift missed.

Employee states he felt lower back pain/discomfort while tramming a 4 yard when he ran over a piece of oversize in road. Went to medic.(this was on his last shift in rotation). On days off, he called & stated his back was still sore-went to doctor on 5/1/2015 & was taken off work.

Employee states he was driving water truck down ramp, when he started feeling dizzy - then his head contacted inside of cab, resulting in laceration above right eyebrow. 3 sutures required. No Days Lost. Full Release issued.

Operator was travelling backwards in haul truck down the ramp, when the haul truck's rear tire contacted the bank causing it to tip on its side. Employee was wearing seat belt and all proper PPE.

Employee stated that he was traveling in haul truck when the truck contacted the rib causing him to make contact w/door frame, resulting in laceration to lower lip & 2 chipped teeth

Employee was mucking out muckbay with skid steer when he hit some hard bottom with bucket causing him to be jerked forward, straining left shoulder. Doctor issued restrictions.

While operating a walk-behind skid steer, employee was walking backwards when his left arm contacted a rear support beam on TBM, causing a contusion, x-rays positive for fracture to left wrist.

Employee stated that while operating a haul truck, he ran over a rock & it bounced him in the seat. Later in shift, he stated he felt discomfort in his neck. We took him to ER-Full Release was issued. On 6/26/2014, he went to another doctor, who issued restrictions. 1st shift missed was 6/27/2014.

Employee was involved in train derailment & received laceration to chin, which required sutures.
