employee was installing a new hose at quarry dewatering pump. A front end loader with forks was being used to position the hose and when the loader back away from the area, his front tires dropped off the work platform and the forks came down on the employees foot. employee had fractured bone in his right foot.

Mostrando los 9Employee slipped off railroad coupler while manually releasing the hand brake on a railroad car. The employee fell against the coupler landing on the left side of his chest against the coupler. The employee was diagnosed with a fractured rib.
Employee was cleaning at tail section of conveyor belt k and his left glove got caught on a trough idler and pulled his left arm in through the idler to his forearm. the left arm was squeezed between the idler and the conveyor skirt frame. He received abrasions to the left forearm.
Employee was driving locomotive with loaded rail cars attached, going toward other cars to hook together when brakes failed. Locomotive traveled 1/4 mile on a 3 slope when it hit other cars.
While pushing and pulling lever to raise and lower truck body, feeling pain thru left arm and elbow.
While operating 980 G loader, a particle of dust or dirt entered his right eye.
WHILE STEERING & SHIFTING LOADER, EMPLOYEE EXPERIENCED SHARP PAIN IN LEFT SHOULDER.
EMPLOYEE REPORTED TO MANAGEMENT ON 8/26/02 THAT HE HAD HURT HIS BACK ON 8/23 & THAT HE WAS GOING TO SEE A DOCTOR. MANAGEMENT SENT HIM TO THE COMPANY DOCTOR AT READING CONVENIENT CARE. HE SAID THAT HE HIT A BUMP IN THE ROAD WHILE DRIVING THE TRUCK & HURT HIS LOWER BACK.
EE WAS CHECKING FAN BELT SLIPPING AND MECHANIC WENT TO START LOADER. SOUNDED WARNING BUZZER, BUT INJURED EE DID NOT REACT QUICK ENOUGH AND GOT FINGER CAUGHT IN BLADES OF FAN BELT.
