Employee operating a yard loader backed into a triaxle truck. Employee waited days later before reporting injuries to supervisor and was assigned to restricted duty beginning 10/26/2009.
Employee was driving a tractor-trailer, loaded with fines, down into the pit, as he reached the intersection of the "New Ramp" and "4th Level Ramp" the tractor-trailer overturned into the bank. The employee was pinned in the overturned cab and had to be removed by emergency personnel. The employee sustained a fractured pelvis.
