Employee was on forklift backing down ramp after dumping a bag of feed in the Rollermill. The forks were extended up with the empty sack on the forks. He went down the ramp at an angle catching the corner of the ramp. This caused the forklift to tilt over. He was complaining of chest, back, and left thigh pain. He also said right eye was blurry.

Mostrando los 3Employee was working on the main chain on a forklift mast. He stood on the dash board of the forklift with his body positioned between the roll cage and mast. While he was working, his foot engaged the tilt back lever pinning him against the roll over cage. A co-worker saw what happened and tilted the mast back to free him.
