Employee noticed idler that was frozen and attempted to free this idler up using a pry bar, the prybar was grabbed by the running Conveyor and pulled in between the trougher idler and the Conveyor belt, this caused the base of the prybar to smash the employee's left hand between the frame and the Prybar.

Employee working on haul truck 552 when right index finger was struck by moving air conditioning belt. Causing a scratch/abrasion.

Employee was sitting behind driver in van 958 when the driver of the van hit a rock and employee who was seat belted into the seat struck their hardhat on a roof of van. Reported back and neck pain. On 12/12/2023 employer notified that pain is related to the work injury and not preexisting condition. Diagnosis cervical strain.

Employee was riding in the back of ASARCO bus when the bus hit a bump and employee was uplifted from seat came back down onto the seat in front striking abdomen on bar/frame.

When tramming 20037 motivator and tractor down to the bottom of the pit, the motivator brakes got hot and spongy due to the operator laying on the brakes too long when going down haul road.

Employee was placing a pan on the apron feeder. When positioning the pan it caught left glove and pulled the hand toward the pans pinching left hand just below the 5th digit causing a laceration. Employee received 5 sutures.

While moving rail, rigging slipped allowing rail to land on foot. Preliminary indications is 3 broken bones

Employee was operating haul truck, haul truck rolled back while employee was on deck causing employee to grab hand rail and hurt right shoulder.

Employee was being transported in a bus to end shift. As the bus was traveling, it encountered a series of bumps that caused the bus to bounce. Employee was propelled off EE's seat impacting the top of the seat in front of EE due to EE not wearing a seat belt.

Employee was using a lift to moving power cable up to the cross member when it came loose and pinched EE's finger against the handrail on the lift.

Employee was moving man basket with a forklift, the man basket base/weight was not secured with the rods that hold the basket to base. The basket tipped off of the base and fell forward toward the truck and hit a piece of pipe that was laid across the bed causing the pipe to flip up and hit EE on the right side of head & knee.

Employee reports while driving haul truck backing into #2 shovel truck struck pothole at same time the bucket was loading the dump bed. Driver felt pain in lower back. On 3/12/2021 the company received notification that a lumbar strain is the diagnosis of injury that occurred on 12/26/2020 and is attributed to a work injury. Lumbar strain is fully resolved.

At 7:45 pm EE was loading the first truck of the shift. When EE went into the dig face for the second bucket, the bucket skipped off a boulder and jarred the loader whipping EE around in the cab. Initially EE didn't think much of it but as the pain turned into spasms that's when EE called to report it. EE reported this incident to me at 3:15 am.

EE was working on the A/C unit on Bus 2 EE removed and re install Blower Motors when EE turn it back on the bus got full of dirt from the unit and it got into EE's eyes prescription eye drops given.

Employee was operating the 532 Loader, and attempting to bring down a section of overhanging material. When the overhanging material broke loose and fell into the bucket the rear-end of the loader lifted off the ground then came back down abruptly. When this happened employee was jolted up and down in the operator's seat.

Employee was installing a new motor on the ditch 154. The motor was hooked to the extended fork lift. As they were lowering it into position, the motor slipped off the mounting plate and caught employees right thumb.

Employee was washing WT508 and turned the water valve off and it broke off and the water pressure knocked employee down causing a fracture to left arm.

The trip cable on ES01 caught on RT560 pulling the sweetheart from the banjo, causing the sweetheart to break free and go through the back window striking the operator.

Employee was casting at the CR4 stockpile. When he curled the bucket back, it hit hard and caused the loader to shake, jarring his back.

Employee was riding as a passenger in VN975. The van was coming up the hercules ramp when it hit a pothole in the haul road and jarred the occupants of the van.

Right tibia was fractured/employee was moving manlift to work site. He hit a bump in the road causing basket to lift up quickly. employees feet came off a basket of manlift when he came down he stuck his feet to the basket causing a fracture to his right tib.

Employee was getting on the bobcat at the load out tunnel, he grabbed the right side safety handle and stepped on to the bucket. His left foot fell between the bucket and loader. Employee began to fall. He pulled hard on the handle pulling a muscle in his right shoulder. Dr ordered physical Therapy 3/28/2013

Employee hit a rut while operating a scraper. The employee failed to adjust and set the air-ride seat. Employee injured his lower tailbone area.

HT operator was injured when the truck he was operating skidded into and struck the rear of a haul truck stopped on a ramp.

Employee states he was injured due to a jarring motion while driving a Haul truck.

Employee was driving haul truck, and he began the dumping process. When the bed reached 4' raise, the hoist cylinder failed causing the bed to lower abruptly. This caused a jarring motion to the employee. He felt no pain at the time, but later began to have stiffness in the upper back and neck. Resulted in neck strain and thoracic strain.

Employee alleges that while driving Haul Truck #380 through the Michelin Hair pin turn the driver's seat suspension bottomed out. This caused pain in his upper back area.

The operator was jarred while being loaded. This injury occurred on 05/08/2011 but did not begin losing time until 05/24/2012. (1 year later)

The operator of water truck 498 lost control and struck a berm. The water truck flipped over. Accident still under investigation.

Employee was driving haul truck, and placed his hand under the arm rest to open the window. At that same time he struck a pothole shifting him to the opposite side of the truck, jarring the left arm and shoulder resulting in left shoulder sprain.

Employee was getting ready to cut vacuum hose. Supervisor told him to wait until truck was turned off. Employee continued to cut hose and in doing so got his face too close to end of hose. His face was sucked to the end and remained until truck was turned off - about 30 seconds.

The investigation is not complete, but as of now it appears a haul truck struck a pick-up truck. The cause of the accident is unknown at this time.

Employee was driving the upper hoist pin out of haul truck 408, wearing leather gloves. When the pin came out of the hoist cylinder the cylinder came down, catching the drift and slamming it in the bed. Employees pinki finger on left hand was caught between drift edge and tapping hole. This caused a laceration to the lateral aspect of fifth digit. (pinki).

Employee was driving passenger out to work from Pit operations building. Employee placed the van in reverse and felt the brakes grind, he then placed the van back into drive to park and down vehicle. When placed in drive the van jumped the curb, and hit the building. Passenger felt back pain and was sent in for medical treatment. Diagnosis back pain.

Employee was driving 900 pit truck to the 5 dump when he struck a pothole. This bounced employee striking his head on the roof of the cab. Resulting in a cervicle muscle strain.

Employee was assisting in a wheel motor change out on HT443, when the wheel started to slide on the wheel chock. This causing the wheel chock to come out from the tire striking the employee in the upper leg and groin area, causing a contusion to the leg and pubic area.

While inspecting the gathering belt to determine serviceability of idlers, EE attempted to free an idler and as he reached in his glove was caught by the belt pulling his hand in up to the middle of his lower arm. Resultant injury was contusion, hematoma and abrasions to the forearm.

Employee was driving haul truck when he encountered a series of potholes that bounced the truck causing him to strike left arm on armrest jarring left shoulder. This reulted in a left shoulder strain.

Employee was backing up haul truck to the shovel. The truck struck the bucket of the shovel. This caused jarring of the truck and employee which resulted in a neck and shoulder strain.

Employee was driving haul truck getting loaded when a large boulder struck the tail end of the truck. It jarred the truck and the employee resulting in a neck sprain.

was assisting in removal of a truck that was stuck. standing behind truck driver pressed the gas pedal sending rocks flying from under the tire. one rock struck left eye brow causing a laceration.

While driving to service a drill, employee encountered a wet down hill grade. He lost control, hit a berm and laid the lube truck #23296 on its top. He received a contusion to the leg and hit his head. Non-work related issues prevented him from returning to work. Recently an IME came back on 12/15 and officially took him off work.

EE operating the 243 Locomotive. He reported as he was approaching the concentrate hopper cars he went to apply the brakes & they did not work. He hit the hopper cars at approx. 5mph causing him to hit the control panel with his body, two facial cuts below his left eye from his safety glasses & he said his right chest area was sore.

EE was assisting car loader in moving rail cars, when the track mobile hit one of the stationary rail cars. EE had contusions to both knees and a sore back.

Employee was on haul truck backing into ES-17 when the bucket came down and hit the back end of the bed jarring employee. **EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 07-26-2008

Employee noticed a Skirtclamp that was dragging on a conveyor belt. Employee (while belt was running) pulled pin from clamp to adjust clamp. While adjusting clamp, trailing edge caught belt which caused it to jump foward pinning employees left hand between the skirt board and clamp. Employee sustained laceration with tendon, ligament and nerve damage.

Employee was hauling to dump down a long ramp. Light rainfall during the shift had caused the ramp to become slick and employee slid into berm. Employee noted pain in left shoulder.

While traveling on the bus at shift change, operator was bounced several times in the seat when the bus went over potholes in the road.
