EE positioned company f-150 truck behind a reversing 980M loader without alerting or making any eye contact with the loader operator. The counterweight on loader struck the right front fender but pickup was drivable. No injury was reported by EE the day of the incident nor the next day. The first report of an injury by EE was late the next day. January 17,2017.

AS HOUSING CAME OUT EE HAD HAND ON LIFTING STRAP & THE HOUSING SHIFTED PINCHING EE'S LEFT HAND BETWEEN STRAP & SHEAVES ON CLUTCH HOUSING. INJURY WAS CONTUSION TO LEFT HAND REQUIRING MODIFIED WORK RESTRICTIONS. NO LOST TIME.
EE WAS STANDING ON A CONVEYOR APPROXIMATELY 5' OFF OF GROUND OBSERVING THE TRANSFER POINT OF THE CONVEYOR HE WAS STANDING ON AND THE DISCHARGE END OF THE CONVEYOR DUMPING ONTO IT. HE LOST HIS FOOTING & GRABBED DRIVE PULLEY OF THE OVERHEAD CONVEYOR CUTTING HIS RIGHT RING FINGER & TWISTING HIS WRIST. THE CUT TOOK 4 STITCHES.
EMPLOYEE WAS REMOVING OVERSIZE ROCK OFF FEED TUNNEL BELT, ROCK SLIPPED FROM EE'S GRIP AND SMASHED RIGHT INDEX FINGER AGAINST OTHER MATERIAL ON CONVEYER BELT.
