Kiln Helper was operating a loader in the coal yard, the loader lunged forward irritating a pre-existing condition. The kiln helper requested not to see a Dr. at the time of the accident. On 6/24/2025 EE went to see the Dr., EE pulled a muscle and was proscribed a muscle relaxer.

Mostrando los 16Employee reported they were in the man-lift and man-lift basket got stuck on a piece of concrete. When the basket broke loose from the concrete it knocked them off their feet and they hit their head on a piece of steel causing a laceration on forehead. They report wearing their hard hat. Employee was taken to the ER for evaluation, they required stitches to the forehead.
EE was driving haul truck from upper pit. There were ruts in the road from the rain the night prior. On EE's way down the ramp, EE hit one of the ruts, it jostled EE around, and EE felt discomfort in EE's back. On Sunday, June 19th, EE went to ER and they prescribed steroids and muscle relaxers and wrote EE out till Wednesday, June 22nd, making it reportable.
While on the lower pier EE was holding the chain from the crane that was wrapped around the housing. While doing so the chain rolled causing EE's hand to roll with it, causing EE's hand to get caught between the chain and the housing.
EE changed the rear diff fluid in water truck at shop. EE checked the brakes and low pressure light in the cab to make sure they had pressure. On the way back to the pit, EE stopped several times and knew they had brakes. At some point on the pit road, EE was unable to stop and wrecked the truck at the bottom. EE was fine, but the doctor kept EE out on Monday.
Employee was operating a JLG man lift. EE had just emptied some kiln leaf seals from the basket into the dumpster. Traveling back to the job site, they ran over the cut out trench that was designed for water drainage, and when the JLG accelerated their feet lifted from the basket platform. When they came down, chin hit the control panel bar. Also feet are bruised.
Employee was traveling in man lift basket. Employee hit a pot hole, they lifted from the man lift basket platform. When they landed, they twisted their left knee.
Employee was clearing lime dust material from a hopper. EE had stepped back while the hopper's vibrator was energized. Suddenly the material inside broke loose splattering EE in face and body area. Apparently some material got behind EE's safety glasses into eyes. EE may have rubbed eyes before using eye wash.
A couple weeks ago he had a crick in his neck. It went away but stayed irritated up until 08/14/12, this is when he reported to his supervisor that his neck was pain. The loader pins are worn causing the bucket to jolt his neck back and forth. The loader was taking out service on the 15th an the bucket pins replaced.
Operator was on the kiln firing deck observing the material inside the kiln. His helper sounded the horn on the front-end loader, where at this time the operator has instruction to go inside the control room for safety. The helper was removing hot rock from underneath the firing deck platform. Hot steam came up and burned the operator.
Process of rolling a tank to reposition. EE was told to move his chock since it was loose. There was a manway in center of the tank so had to put the forklift on the other end of the tank to releive pressue from bound end to remove that chock. At the same time the bound end had been freed the forklift was backing off tank to reposition. Tank rolled & pinned EE R Leg.
EMP THOUGHT THE BRG. WAS LOCKED UP ON THE 2 DECKSCREEN. GRABBED THE V BELTS TO ROCK THE SCREEN WHEN IT ROLLED UNEXPECTEDLY CATCHING HIS FINGER BETWEEN THE V BELT AND SHEAVE. 5 STITCHES.
EMPL WAS TRYING TO FREE A STICKY VALVE ON THE HYDRO CRANE TRUCK HE UNSCREWED THE AIR LINE BEFORE THE AIR TANK DEPRESSURIZED EMBEDING SMALL PARTICLES OF OIL IN THE SKIN BETWEEN THUMB AND INDEX FINGE R HAND SUBCUTANEOUS
EE WAS RUNNING LATE FOR WORK HE STOPPED HIS TRUCK IN FRONT OF OFFICE. GOT OUT WHEN HIS TRUCK STARTED BACKWARD AND THE DOOR KNOCKED HIM DOWN THE LEFT FRONT WHEEL RAN OVER HIS LEFT FOOT. EE DID NOT GET TO DOCK IN FOR HIS 1130 SHIFT.
EMP WAS ASJUSTING CHUTE SKIRT WITH HAMMER CONVEYOR WAS RUNNING HIS RT HAND WENT BETWEEN SKIRT AND BELT. HISHAND WAS CUT WHICH REQUIRED STITCHES AND BROKE HIS FINGER THE GUARD WAS OUT OF PLACE, THE CONVEYOR WAS NOT NOT LOCK OUT IN THE OFF POSITION
HE WAS CHANGING ROLLERS ON CONVEYOR, HIS LEFT MIDDLE FINGER WAS MASHED BETWEEN ROLLER AND BELT CAUSING BLOOD POISON UP LEFT ARM.
