Employee was dismounting loader and while stepping down he hit a lose step, twisting his right ankle.

Employee was operating a back dump when he started to feel a dull pain on the left side of his back, neck and shoulder. Pain intensified as time went on resulting in the employee asking to be taken to an emergency room to be checked out.

Ee was operating the 980 ft loader and the bucket slipped off rock and jerked his neck.

DRIVING END DUM1 ON FLAT SURFACE.MUSJUDGED DISTANCE & CAME TOO CLOSE TO EDGE OF ROADWAY.TRUCK TURNED OVER AND HE WAS THROWN FROM TRUCK.DOESNT REMEMBER WHAT HAPPENED ONLY REMEMBERS TRUCK BEGINN ING TO TURN THE3 BEING OUT AND CRAWLING UP EMBANKMENT AFTER TRUCK STOPPED.
