The employee had finished fueling a machine and was driving fuel truck away and stalled the engine. He rolled backward approximately 10 feet and hit the track of the machine. Employee pulled muscles in right shoulder.

HE REACHED OUT OF REDLEY FOR THE HAT WHEN HIS THUMB OFF THE FEEN. HE HAD TO HAVE FOUR STITCHES THE CUT ON HIS LEFT HAND.
EMPLOYEE DRIVES A ROCK TRUCK. WHILE DUMPING HIS LOAD HE RECEIVED A HARD BOUNCE. THIS BOUNCE CAUSED EMPLOYEE BACK PAIN. LATER IN THE SHIFT HE BECAME SICK TO HIS STOMACH.
EMPLOYEE WAS USING A SHOVEL TO CLEAN THE INSIDE TOP OF THE WA500 LOADER BUCKETT, WHICH WAS ABOUT 4 FEET OFF THE GROUND. ANOTHER EMPLOYEE INSIDE LOADER CAB KNOCKED THE TILT GEAR OUT OF PLACE CA USING THE INSIDE TOP PORTION OF THE BUCKETT TO ROLL DOWN AND HIT EMPLOYEE ON TOP OF THE HEAD.
EMPLOYEE WAS OPERATING FARM TRACTOR WITH BUSH-HOG ATTACHED. WHILE CUTTING REVEGETATED AREA, EMPLOYEE HIT A ROCK WITH THE FRONT WHEEL. THIS CAUSE THE STERRING WHEEL TO JERK IN A DIFFERENT DIRE CTION FROM WHICH EMPLOYEE WAS STERRING WHEN IT DID THIS IT JERKED HIS LEFT (ARM) ELBOW.
