EMPLOYEE WAS MOVING SHUTTLE CAR CABLE WITH A SCOOP BY TRAMMING ALONG THE CABLE AND REACHING OUT OF SCOOP WITH HIS LEFT HAND, LOOPING THE CABLE AROUND THE BACK OF DECK. WHILE SCOOP WAS RUNNING , CAUGHT HIS ARM BETWEEN SCCOP AND RIB.
HE WAS BACKING A SUPPLY TRUCK UP WHEN HE WENT TO TURN TO LOOK OUT THE BACK WINDOW & PULLED A MUSCLE IN HIS BACK.
HE WAS ATTEMPTING TO PUT A GREASE HOSE ON THE NO. 2 BELT DRIVE SNUB ROLLER. THE HOSE GOT CAUGHT IN THE ROLLER & PULLED HIS LT. HAND & ARM BETWEEN THE BELT & ROLLER.
EMP WAS CLEANING #7 ROADWAY AND WAS GETTING READY TO ROCK DUST WHEN A PIECE OF ROCK FELL FROM BETWEEN THE BOLTS AND RIB AND HIT HIM ON HIS NECK,SHOULDER AND BACK.
WHILE OPERATING A SHUTTLE CAR HE SAID HE RAN OVER A ROCK IN THE ROADWAY AND THE SHUTTLE CAR BOUNCED AND FELT A PAIN IN HIS NECK.
EE WAS UNLOADING SUPPLIES WITH A SCOOP. PICKED UP A PALLET OF ROCKDUST - THE SCOOP BOWED UP - ROOFED THE EE.
EE WAS OPERATING 2 SCOOP IN 2 ENTRY THE SCOOP WHEEL DROPPED INTO A HOLE CAUSING EE TO HURT HIS BACK. WATER WAS IN ROADWAY IN ABLING HIM TO SEE THE CHUG HOLE.
EE WAS PUSHING A POWER BOX IN PLACE WITH A SCOOP. THE SCOOP TIRE PULLED ON A CRIB BLOCK. THE CRIB BLOCK FLEW OUT CAUSING HIM TO BOUNCE & HIT HISBACK ON LOWER SEAT & CHAIN.
EMP WAS TROUBLE SHOOTING A J B LONG BATTERY VEHICLE. HE WAS WORKING IN MAIN CONTROL PANEL AT REAR OF VEHICLE TRACING WIRING WHEN VEHICLE STARTEDAND TRAMMED BACKWARDS, MASHING EMP BETWEEN VEHIC LE AND SECTION TRANSFORMER.
EMPLOYEE STATEDTHAT HE WAS BOUNCED SIDEWAYS IN SEATING COMPARTMENT OF RAIL OPERATED MANTRIP WHILE IN TRAVEL TO HIS WORKING SECTION CAUSING PAIN TO HIS NECK
EMPLOYEE SAID HE HAD HELPED SPLICE SECTION BELT AND RETURNED TO OPERATE HIS SHUTTLE CAR. CLAIMED HIS BACK STARTED HURTING. HE SAID HE DIDN'T KNOWINGLY HURT HIS BACK. HE JUST DEVELOPED THE BACK PAIN DURING THIS SHIFT.
VICTIM A CO-WORKER STOPPED AT THE #8 BELT DRIVE TO LEAVE SOME HOSES FOR A NEW BELT INSTALLATION. THE 3-WHEELER ROADWAY PASSES UNDER THE #8 BELT & THE CLEARANCE IS APPROX 4' HIGH. DURING TH E PROCESS OF LAYING OFF HOSES THE VICTIM POSITIONED HIMSELF ON TH3 3-WHEELS IN A MANNER THAT THE OPERATOR THOUGHT HE WAS READY TO GO AND TOOK OFF CAUSING VICTIM TO STRIKE BELT GUARD.
EMPLOYEE WAS OPERATING A SCOOP AND PULLED ALONGSIDE A ROCKDUSTER, REACHED OUT OF OPERATORS COMPARTMENT WITH RIGHT HAND TO MOVE A HOSE OUT OF ROADWAY AND IN DOING SO, HE MOVED STEERING LEVER OF SCOOP CAUSING SCOOP TO SHIFT AND MASHED HIS HAND BETWEEN SCOOP DECK AND ROCK DUSTER MACHINE.
WERE MOVING 3 FLATCARS OF TRASH DOWN TRACK. INJURED WAS ATTEMPTING TO HOOK UP THE SAFETY CHAIN ON END CAR. SAID HE THOUGHT HE HAD ENOUGHT SLARK IN THE CHAIN TO HOOK IT UP AND WHEN HE DID HE CA UGHT HIS RT HAND BETWEEN THE SUPPLY CAR AND SAFETY CHAIN.
INJURED EMPLOYEE WAS BLEEDING HYD. LINE FROM PUMP TO POWER TAKE-OFF. HE WAS UNDER TRUCK AND SECOND EMPLOYEE WAS IN TRUCK REVING ENGINE & ENGAGED BED JACK. THE SHIFTING LEVER JUMPED FROM NEUTRA L TO REVERSE CAUSING THE TRUCK TO MOVE BACKWARDS 8". EMPLOYEE'S FEET WERE CAUGHT UNDER RT. FRONT WHEEL.
EE WAS LEAVING CONTINUOUS MINES WITH LOAD AND RUN THRU VENTILATION CONTROL AND FLY PAD AT CONTROL HIT HIM IN FACE KNOCKING OFF GLASSES AND HAT STRIKING HIM ACROSS EYE.
EMPLOYEE WAS ASSISTING IN UNLOADING 300 FEET OF LAPPED BELT FROM A FLAT CAR, WHILE PULLING BELT WITH SCOOP AND TRAMMING CAR WITH MOTOR TO POSITION BELT AT BELT HEAD. REAR LAPS OF BELT PINNED E MPLOYEE AGAINST BELT CROSS-OVER LADDER.

VICTIM STATED HE RAN OVER A PIECE OF ROCK ALONG HAULWAY, CAUSING HIM TO BOUNCE UPWARD AND HE LANDED ON HIS SELF RESCUER. HE DIDN'T INFORM FOREMAN UNTIL END OF SHIFT ON SURFACE.
EMPLOYEE WAS IN A MANTRIP AS IT CAME TO THE END OF TRACK.THERE WERE 8 RAILS IN CENTER OF TRACK.,ONE RAIL WAS STICKING UP AND THE END OF IT HIT THE MANTRIP AND SLID UNDER IT WHERE THE EMPLOYEE WAS LAYING.EMPLOYEE SAID HE COULD FEEL THE RAIL GO UP HIS BACK THROUGH THE BOTTOM OF THE TRIP. EMPLOYEE CONTINUED TO WORK UNTIL 05-05-92.
