EMPLOYEE STATES WHILE DRIVING THE SCOOP HE RAN OVER A MEGA BLOCK CAUSING THE SCOOP TO BOUNCE.EMPLOYEE STATES HE WAS BOUNCED AROUND IN THE SCOOP STRAINING HIS LOWER BACK AND LEFT HIP.EMPLOYEE W ENT TO THE DOCTOR ON 07-31-90 AND WAS PLACED ON RESTRICTED DUTY 08-01-90.

EMPLOYEE STATES WHILE HE WAS WATCHING THE FEEDER CABLE, THE OPERATOR OF THE FEEDER ACTIVATED THE HYDRAULIC HOSE. THE HOSE HAD A BROKEN FITTING AND FR FLUID SPRAYED OUT HITTING EMPLOYEE IN THE LEFT EYE. ON 12-14-89 THE EYE STILL APPEARED TO BE IRRITATED FROM THE FLUID AND EMPLOYEE WAS GIVEN A PRESCRIPTION.
EMPLOYEE STATES HE HAD STOPPED THE TELETRAM TO LET IT COOL DOWN AND FAILED TO SET THE BRAKE. THE TELETRAM ROLLED FORWARD AND HIT A RIB, AND THE EMPLOYEE HIT HIS FOREHEAD. HE RECEIVED A LACERAT ION WHICH REQUIRED SEVERAL STITCHES.
EMPL WALKING TOWARD 5A MAIN INTAKE PORTAL -STRUCK FROM BEHIND BY SCOOP CAR.EMPL ROLLED AWAY FROM SCOOP BUT RIGHT ARM WAS STRUCK BY TIRE ON SCOOP REQUIRING SURGERY.EMPL MAY BE OFF 3-6 MONTHS.
EMPL STATRES HIS SCOOP RAN OVER CRIB LEFT IN ROADWAY & HEAD WAS DRIVEN INTO CANOPY OF VEHICLE. EMPL WAS ISSUED TWO PRESCRIPTIONS.

EMPL. WAS MOVING CRIB WITH SCOOP. BAR SLIPPED & HIT HIS RGHT KNEE. HE WAS EXAMINED BY PHYSICIAN BUT DID NOT MISS ANYTIME FROM WORK. HE CONTINUED TO HAVE PROBLEMS WITH HIS KNEE & REQUIRE D AN ARTHROSCOPY ON 3/2/88.
EMPLOYEE WAS OP9RATING A LST-20X PULLING A CROSS-PIECE FOR PAN LINE. THE STRAIN CAUSED SOME BOLTS TO GIVE WAY, WHICH RESULTED IN THE LST-I/X BREAKING IN HALF. THE JOLT THREW THE EMPLOYEE UP AGAINST THE ROO0, AND HE ALLEGES HE JAMMED HIS NECK. THE PHYSICIAN PRESCRIBED MUSLE RELAXANTS.
EMPLOYEE WAS HAULING RED ROCK IN THE BUCKET OF A SCOOP. HE TURNED THE SCOOP TOO SHARP FOR THE LOAD AND IT LEANED OVER ON TWO WHEELS. WHEN THE SCOOP STTLED BACK DOWN ON FOUR WHEELS, THE JOLT C AUSED A BACK STRAIN.
EMP WAS WALKING3BESIDE FEEDER TO SHOVEL ON THE OTHER SIDE. A TRACK SHIELD SITTING ON TOP OF THE FEEDER FELL OFF AND HIT THE LEFT KNEE. EMP CONTINUED TO WORK, HOWEVER THE LEFT KNEE PROGRESSIVEL Y GOT WORSE. ON311-25-85 PHYSICAL EXAM DIAGNOSED A TORN LEFT MEDIAL MENISCUS. EMP UNDERWENT ARTHROSCOPY AND PARTIAL MEDIAL MENISCECTOMY ON 12-2-85 AND WAS RELEASED TO RETURN TO WORK 12-11-85
VICTIM WAS HANGING A FIRE SENSOR OVER D BELT DRIVE, STANDING ON C TAILPIECE-AS HE WAS CLIMBING DOWN OFF TAILPIECE, HE STEPPED ON THE CHAIN LINK GUARDING AND THE TAIL PULLEY PULLED THE GUARD AN D VICTIM\S LEG INTO THE TAILPIECE.
