Driver was backed up to lime pile to dump. As the bed started up, one side stuck and caused the unit to go on its side. Driver got out through windshield. Was transported to hospital but was released. His side is sore.

Scrapping loose rock from highwall with front-end loader. The bucket slipped off a rock, hit another rock and slammed employee back into the seat causing the injury.

While lifting a concrete barrier with crane the injured reported that he may have hit the wrong control then over-corrected resulting in his right hand being crushed.

While traveling to work in company truck along Rt. 153 near Penfield the vehicle hydroplaned on substance (mud/water/oil) on road resulting in the truck flipping over.

WHILE CLIMBING THE LADDER TO GET INTO ROCK TRUCK, THE ROCK TRUCK DRIFTED AHEAD SIX TO EIGHT FEET PINNING HIM IN THE PELVIS AREA AGAINST ANOTHER ROCK TRUCK.

OPERATING PAN, MOVING DIRT, WHEELS OF PAN GOT BETWEEN TWO LARGE ROCKS CAUSING STEERING WHEEL TO TURN, HITTING LEFT WRIST.

HE HAS BEEN OPERATING 50 TON ROCK TRUCK FOR PERIOD OF TIME WHILE HE WAS OPERATING ROCK TRUCK HE RAN OVER LARGE ROCK WHICH CAUSED TRUCK TO BOUNCE & POSSIBLE INJUR HIS BACK AT THAT TIME. HE CONT INUED TO OPERATE ROCK TRUCK & CONTINUING OPERATION OF TRUCK MIGHT OF CAUSED FURTHER INJURY TO HIS BACK. HE DID NOT MISS DAY OF WORK UNTIL 6/29/89.

HE WAS DRIVING HIS TRUCK WITH THE SIDE WINDOW OPEN TO LOOK AT A DOZER AND THERE WAS A FALLEN TREE ALONG SIDE THE ROAD AND SOME OF THE BRANCHES FROM THE TREE CAME IN THE OPEN WINDOW AND STRUCK HIM IN THE LEFT EYE BALL, CUTTING THE EYE BALL.
