OPERATOR OF 2ND SHUTTLE CAR WAITED FOR EE TO CLEAR THE INTERSECTION. 1ST CAR HAD TO BACK UP A SHORT DISTANCE TO CLEAR THE LEFT RIB. OPERATOR OF 2ND CAR THOUGHT HE HAD CLEARED THE INTERSECTION & PULLED FORWARD STRIKING 1ST CAR.
EMPLOYEE WAS TRAMMING HIS SHUTTLE CAR TOWARD THECONT MINER. THE CAR CAME VERY CLOSE TO THE RIB AND THE EMPLOYEE REACHED OUT OF THE CAR AND INJURED TWO OF HIS FINGERS.
EE WAS OPERATING SCOOP. THE SCOOP BUCKET BRUSHED THE RIB AND A PIECE FELL OFF STRIKING HIM IN THE BACK & SIDE.
EMPLOYEE WAS PULLING TH EMINER CABLE WITH HIS BACK TOWARD THE SHUTTLE CAR. THE SHUTTLE CAR WAS PULLING AWAY FROM THE MINER ON A SLIGHT PITCHED ENTRY, THE LOADING END OF THE SHUTTLE CAR SLID T O THE SIDE AND STRUCK THE EMPLOYEE.
