Employee was driving to the Coal pit and hit something in the road, EE was jarred and said EE's back and leg was hurting. Went to EE's Dr. and was given days off to rest.

Employee was operating a tractor trailer coal truck when they hit a rough spot in the road. Lower Right back began hurting, EE was taken to PMC hospital. The employee was x-rayed, given medicine and taken off work for a strained back.

Employee was operating a rock truck when EE hit a bump in the road and said it jarred EE around. The employee went to the Doctor the next day complaining of back pain. X-ray showed bulging disk, waiting on an MRI.

Employee was operating an 830E Komatsu Rock truck. While being loaded a rock slipped out of the bucket hitting the top side rail of the truck bed jarring the Employee. The employee went to the hospital complaining of back pain and tingling in right foot.

Employee was operating a water truck coming out of the job. Employee did a brake check at the top of the hill, the truck popped out of gear half way down the steep EE lost control and the truck turned over on its driver's side at the bottom. EE was taken by ambulance to the hospital. EE was X-rayed, no broken bones and had stitches in hand, cuts and bruises.

Employee was operating a rock truck, leaving the pit area into the valley fill they hit a bump in the road and said their lower back was hurting. They left around 11:00 PM and went to the doctor the nest day. X-rays where done and they were treated for a strained back.

Employee was coming out of the west cut through pit Loaded when the trailer unhooked from the tractor landing on the rear axle jarring the driver. The employee was taken to the hosptital and treated for neck and back pain.

The employee was hauling a load of coal on a road being worked, after being stopped for a couple of minutes they proceeded on the road. They traveled a short distance when the front tire of the truck encountered a soft area it turned over on the right side. When the employee released their seat belt, EE landed on their right shoulder before exiting the truck.

Employee was traveling out of the parking lot in a pickup truck beside a rock truck, when the rock truck made a right turn, hitting the front end of the pickup. It pushed the pickup truck to the right, jerking the employee around.

Employee was scraping mud off the haul road with a front end loader when EE hit a rock with the bucket. It stopped EE immediately and the loader twisted sideways resulting in a strain to EE's lower back.

Injured pulled EE's personal vehicle within 5' of a contract welders service truck. Welder was pulling off start line when injured got out of EE's car (which was lower than the hood of the truck) the truck pulled off the start line striking the injured's personal vehicle door causing it to strike the injured resulting in fracture to the top of the tibia bone.

Driver said the truck jumped out of gear causing the truck to run away down the hill. Driver has 6 cracked ribs and a punctured lung and is recovering at home. MSHA investigators have inspected the truck and found no problems with the truck transmission.

Employee was operating a swivel truck and allegedly hit a hole in the haul road and allegedly jarred back.

He was coming down a hill in a coal truck. He lost control of the truck and turned it over.

Driver was coming from the deep mine with a load and lost control at truck came from #18 to #17 marker and dropped off into a sump.

Employee was hauling and dumping rock in valley fill - he backed truck up too far and slope gave way - truck went back over embankment about 40 - 50 ft - truck turned over.

HAULING COAL DOWN HILL, LOST BRAKES AND HIT ANOTHER TRUCK FROM THE REAR RESULTING IN LIFTINGIT UP AND TURNING IT OVER.

EE WAS HELPING TO CLEAN RAILROAD CARS. AFTER CLEANING CARS HE BEGAN RUNNING LOAD OUT SECTION OF TIPPLE AND ALLEDGED SHARP PAIN IN LOWER BACK AND NUMBNESS IN LEGS EE CONTINUED TO WORK UNTIL 1-2 -96 WHEN HE MISSED WORK.

EMP ALLEGES THAT WHILE RUNING A FRONT END LOADER THAT HIS BACK BEGAN TO HURT OVER A PERIOD OF TIME. THERE WAS NOT ANY SPECIFIC ACCIDENT THAT CAUSED THIS CONDITION. BILLING 1ST REPORTED TO BLUE CROSS LATER TO WORKMENS COMP.

EMPLOYEE HAS BEEN ABSENT FROM WORK SINCE THE ABOVE DATE. HIS DOCTOR ALLEDGES AS OF 9/5/94 THAT EVEN THOUGH NO SPECIFIC ACCIDENT HAS OCCURED THAT DUE TO DRIVING A ROCK TRUCK ON SURFACE OPERATI ON EMPLOYEE HAS INJURED HIS BACK.

EMPLOYEE WAS OPERATING A FRONT END LOADER MOVING THE BUCKET INTO A DIRT PILE. HE HIT A ROCK INT HEDIRT PILE WITHT HE BUCKET CAUSIGN THE LOADER TO JAR, EMPLOYEES BACK. HOWEVER EMPLOYEE CONTIN UED TO WORK UTNIL 2-23-94 WHEN HE ALLEDGE PAIN GREW WORSE.

EMPLOYEE WAS OPERATING ATRACK FRONT ENDLOADER ATTEMPTING TO SMOOTH A ROADWAY. WHILEREVERSING LOADER HE BACKED INT OHIGHWALL WITH SUDDEN IMPACT CAUSING EMPOYEE TO JERK NECK AND BACK IN A WHIPL ASH MOTION. SEATBELTS WERE BEING WORN.

EMPLOYEE ALLEDGES THAT ON 01-1 5-93 WHILE DRIVING A 777 ROCK TRUCK THAT HE FELT SOMETHING PULL IN HIS BACK HE CONTINUED TO WORK UNTIL 08-18-93 WHEN HE ALLEDGED THAT HIS BACK DIDNT GET ANY BETT ER & HE NEEDED TO SEEK MEDICAL ADVICE

MECHANIC WAS PULLING TRACK OVER THE IDLER WITH BOOM TRUCK TRACK GOT HUNG ON IDLER MECHANIC ATTEMPT TO UNFACE TRACK IT CAUGHT HIS RT FOUR FINGER BETWEEN TRACK & THE IDLER & CUT IT

EE WAS ASSISTING A MECHANIC IN LOADING THE FRONT SECTION OF ROLLER FRAME OF A DOZER WHEN EE WAS GUIDING ROLLER FRAME INTO BACK OF PARTS TRUCK WHEN THE BOGGIE OF THE ROLLER FRAME CAUGHT THE TIP END OF LEFT RING FINGER. EE NETURAL TO WORK ON 12/23/92.

EMPLOYEE WAS BACKING ROCK TRUCK TO DUMP INTO HOLLOW FILL. WHEN TRUCK BACKED TO DUMP MATERIAL HOLLOW FILL GAVE WAY CAUSING TRUCK TO FALL BACK ON ITS TOP.

THE CAUSE OF THE INJURY IS UNCERTAIN. EMPLOYEE WAS OPERATING A FRONT END LOADER WHEN HIS LEFT LEG BECAME PAINFUL. EMPLOYEE ALLEDGES PAIN BECAME SO BAD THAT HE WAS UNABLE TO WORK. NO ACCIDENT T RAMA OCCURRED THIS IS POSSIBLY TIED TO A 1985 BROKEN HIP" INJURY.
