THE INJURED WAS ASSISTING A SCOOP OPEPRATOR LOAD A BELT TAKE-UP UNIT ONTO A SUPPY CAR. AS THE SCOOP OPERATOR WA SRETRACTING THE BLADE OF THE SCOOP. THE BLADE PINCHED THE INJUREDS LEFT HAND BE TWEEN THE RETRACTABLE BLADE AND THE SCOOP BUCKET.
EE WAS OPERATING A RAIL JEEP AND FAILED TO LOWER HIS HEAD FOR A STEEL BEAM, THEREFORE HE HIT HIS HEAD. THE AREA IS IN AN IDLE PART OF THE MINE AT #4 ROOM OVERCAST ON 2 SOUTH HAULAGE.
EMPLOYEE WAS INSTALLING A DRAW-BAR FOR A BELT CAR IN-PLACE-OF A COUPLER ON A LOCOMOTIVE. WHEN HE WAS INSERTING THE PIN BACK IN THE LOCOMOTIVE THE LOCOMOTIVE WAS MOVED TO ALINE THE HOLES BETTER , BUT HE FAILED TO MOVE HIS HAND OUT OF THE WAY.HE PINCHED HIS FINGER UNDER THE PIN.
EE WAS A PASSENGER IN A MANTRIP. AS THE MANTRIP PASSED THRU A STEEL HAULAGE DOOR, THE UNATTENDED BEGAN TO CLOSE. THE OPERATOR STOPPED TO REACH OUT TO PUSH THE DOOR BACK OPEN, BUT BEFORE HE COU LD, THE MANTRIP DRIFTED BACKWARDS AND THE BOTTOMCORNER OF THE DOOR CAME INTO THE PASSENGERS' COMPARTMENT AND SPEARED THE FOOT OF VICTIM.
EMPLOYEE INADVERTENTLY LEFT HIS HAND ON THE RAM PART OF A SCOOP BUCKET WHILE THE RAM WAS MOVING TO ITS IN LOCATION WHILE LOADING ROOF BOLTS ON THE BUCKET HE SAID HE WAS HOLDING THE CHAIN IN PL ACE THAT WAS PULLING THE BOLTS ON HIS FINGERS WERE PINCHED BETWEEN THE RAM & THE BUCKET
EMPLOYEE WAS PICKING UP SLATE SPILLAGE AND THROWING IT ON THE CONVEYOR BELT. A PIECE OF SLATE FLEW BACK STRIKING HIM IN THE EYE.
EMPLOYEE WAS PUTTING PIN INTO COUPLING DEVICE ON LOCOMOTIVE & DRAW BAR WHEN THE PIN WOULDNT GO IN HE HAD OPERATOR BUMP LOCOMOTIVE CONTROL TO GIVE SLACK BUT WHEN HE DID THE PIN CAUGHT HIS FINGE R AGAINST LOCOMOTIVE BUMPER
THE INJURED WAS A PASSANGER ON A MANTRIP THE DRIVER OF THE MANTRIP WAS SWITCHING INTO A SIDE TRACK AND RAN INTO ANOTHER PARKED PERSONEEL CARRIER. THE INJURED SUSTAINED A WHIPLASH TO HIS NECK A ND BACK.
EMPLOYEE WAS OPERATING A UNI HAULER WHEN IT RAN OVER A CONCRETE BLOCK LYING IN THE ROADWAY CAUSING THE EMPLOYEE TO STRIKE HIS HEAD ON THE CANOPY AT WHICH TIME HE EXPERIENCED A PAIN IN HIS NECK HE CONTINUED WORKING HIS NORMAL DUTY UNTIL 04-007-92 WHEN HE WAS PLACED ON RESTRICTED WORK DUTY
EMPLOYEE WAS OPERATING A UNIHAULER WHEN IT STRUCK A BUMP IN THE ROADWAY CAUSING THE EMPLOYEE TO EXPERIENCE A PAIN IN HIS LOWER BACK.
EMPLOYEE WAS STANDING ON THE GUNWHALE OF A BARGE. WHEN THE OTHER EMPTY BARGE STRUCK THE ONE THE EMPLOYEE WAS STANDING ON, IT KNOCKED HIM OFF BALANCE CAUSING HIM TO FALL INTO THE EMPTY BARGE, U PON WHICH HE SUSTAINED A BROKEN ARM AND FRACTURED RIBS.
INJURED WAS ATTEMPTING TO UNCOUPLE SUPPLY WAGONS WHEN HE PUSHED DOWN ON THE HANDLE THE PIN CAME OUT ALLOWING THE HANDLE TO COME UP & STRIKE HIM IN THE FOREHEAD CAUSING A LACERATION
THE INJURED WAS OPERATING A BATTERY POWERED RAM CAR WHEN HE HIT A BUMP ON THE TRAM ROAD AND INJURED HIS LOWER BACK.
INJURED WAS OPERATING A UNIHAULER WHEN IT STRUCK A HOLE IN THE ROADWAY AND CAUSED INJURED TO STRIKE HIS HEAD AGAINST THE CANOPY OF THE UNIHAULER AND HE SUFFERED A STRAIN TO HIS NECK WHEN HE BO UNCED BACK INTO THE SEAT.
THE INJURED WAS IN THE PROCESS OF TRAMING THE STAMLER CRUSHER FEEDER OFF THE TAILPIECE. WHILE RAISING THE BOOM OF THE FEEDER A HYDRAULIC HOSE BROKE AND OIL SQUIRTED INTO THE INJURED S EYES.
