WHILE EMPLOYEE WAS UNCOUPLING A CARE HE SMASHED RIGHT INDEX FINGER. EMPLOYEE RECIEVED MEDICAL TREATMENT ON 6/22/93. HE DID NOT REPORT THE TREATMENT UNTIL 7/6/93 WHEN OPERATOR CONTACTED HIM @ HOME DURING VACATION. FIRST DAY MISSED 7/12/93.
WHILE THE EMPLOYEE WAS OPERATINGA SHUTTLE CAR HE INJURED HIS LOWER BACK. THIS ACCIDNET WAS NOT REPORTED UNTIL 11-16-92. WHEN MANAGEMENT CALLED THE EMPLOYEE AT HOME.
WHILE THE EMPLOYEE WAS SHOVELING NO.7 BELT TAILPIECE, THE SHOVEL. HIT THE TAILROLELR, IN WHICH GLANCED BACK AND HIT HIS RIGHT KNEE. NOTE: THE EMPLOYEE DID NOT REPORT THIS TO THE OPERATOR UN TIL 7-13-92.
THE EMPLOYEE WAS LEANING DOWN IN FRONT OF THE 3-WHEELER, WENT TO TURN AND SET ON THE FRONT OF IT WHEN THE REPAIRMAN STARTED MOVING IT IN REVERSE. THIS CAUSED HIM TO FALL STRIKING HIS BACK ON T HE THREE-WHEELER. NOTE: THE FIRST DAY MISSED WAS 6-4-92.
ONE END OF A HEADER HAD FALLEN AND WAS HANGING DIAGONAL ACROSS THE TRACK ENTRY. WHEN THE TRACK MANBUS WAS PASSING UNDER IT, THE HEADER STRUCK THE EMPLOYEE IN THE BACK OF THE NECK.
THE INJURED EE WAS HAULING GOB FROM A NEW HEAD DRIVE LOCATION. HE LOST PART OF THE GOB AND THEN HE DROVE UPON IT AND THEN GOT THROWN INTO THE MINE ROOF.
INJURED PARKED HIS COALHAULER IN A BLID AREA OF THE HAULACE TRAVELAY, ANOTHE RCOALHAULER HIT HIM. THIS PUSHED THE PARKED COALHAULER INTO THE RIB AND RESULTED INA CONTUSION TO HIS LEFT ARM. N OTE THE EMPLOYEE REPORTED TO WORK ON 11-13-91 AND MISSED THE NEXT TWO DAYS, HE DID NTO REPORT THE REASON UNTIL 12-10-91 WHEN QUESTIONED.
WHILE THE EMPLOYEE WAS RUNNING THE OPEN TYPE MANBUS HE GOT HIS RIGHT SHOULDER CAUGHT AGAINST THE MINE ROOF, THUS CAUSING A CONTUSION TO HIS RIGHT ARM.
WHILE RIDING MANTRIP A DROP OF WATER HIT THE EMPLOYEE IN THE LEFT EYE.
