WATER TRUCK DRIVER WAS INJURED WHILE DRIVING OVER A ROUGH SECTION OF ACCESS ROAD WHEN HE BOUNCED AND STRUCK HIS HEAD ON THE ROOF OF THE TRUCK. THIS RESULTED IN BACK PAIN WHICH TEMPORARILY IMMO BILZED THE DRIVER. HE WAS EXPERIENCING PAIN IN HIS BACK AND RIGHT SIDE.

INJURED STATES THAT AS HE WAS TRAVELING DOWN #3 ROADWAY 001 SECTION TOWARD FEEDER A PIECE OF STAGING (STRING) USED IN SPAD ALIGNMENT CAUGHT ON SIDE BOARD OF CAR AND BROKE AS IT BROKE IT SLIPPED INTO HIS RIGHT EYE CAUSING A CORNEAL ABRASION.
INJURED STATES THAT AS HE ENTERED THE LAST OPEN BREAK #3 INTERSECTION OF 003 SECTION ON SHUTTLE CAR, HE RAN OVER A PIECE OF ROCK IN ROADWAY. STATES THAT THE ROCK EITHER FELL FROM HIS OR THE ST D CAR ON PREVIOUS TRIP. ROCK WAS NOT THERE APPROX. 4 MINS BEFORE. COMPLAINS OF LUMBAR-SACRAL PAIN. X-RAY INDICATED SLIGHT WEDGING OF T-12 POSSIBLY OLD INJURY.
ACCORDING TO INJURED PERSON HE HAD HIS HAND ON THE EDGE OF THE OPERATOR DECK. WHEN HE ENGAGED REVERSE TRAM THE DECK SHIFTED SLIGHTLY CATCHING HIS LEFT SMALL FINGER THIS WAS REPORTED TO HAVE OC CURRED IN THE 5 ENTRY OF 003 SECITON. INJURED WAS OFF WORK 3-23-95 MINES WERE IDLE 3-24 DUE TO CONTRACT DELIVERIES MISSED ONE DAY 7 SUTURES.
ACCORDING TO INJURED, DURING MANTRIP FROM 003 TO SURFACE, THE SCOOP OPERATOR EITHER INADVERTENTLY ENGAGED THE RAM "IN" LEVER WHILE TRAMMING THE SCOOP. THE INJURED PERSON HAD UNKNOWINGLY PLACED HIS FINGERS IN THE PINCH POINT BETWEEN RAM AND MAIN BUCKET. X-RAY REVEALED SIMPLE FRACTURE TO DISTAL MIDDLE AND RING FINGER. BUCKET RAM MAY HAVE SHIFTED SLIGHTLY ON BUMP.
EE STATES THAT HE WAS NEAR A SHUTTLE CAR WHICH WAS NOT RUNNING. THE CAR OPERATOR UNEXPECTEDLY STARTED THE MACHINE AND INJURED STATES SLACK OF CABLE JERKED HIS FEET OFF MINE FLOOR. HE STATES TH E HE LANDED ON HIS LIGHT BATTERY & RESCUER INJURING LOWER BACK. RT. KNEE ALSO INJURED.
INJURED STATES THAT AS HE DROVE THE SHUTTLE CAR TOWARD THE FACE IN #3 ENTRY HE DROVE ACROSS A CRIB BLOCK LAYING IN THE ROADWAY. CAUSED A BRUISE/TRAUMA TO LWOER BACK LEFT SIDE.
INJURED STATES THAT AS THE RAILRUNNER(MANTRIP) DROPPED OFF THE RAIL AT 100 BREAK RIGHT SIDE NEAR SPINE.
ACCORDING TO STATEMENT OF INJURED THE WEST VA. ARMATURE RAIL RUNNER HE WAS OPERATING STUCK IN FORWARD POINT & HE JUMPED OUT ONTO MINE FLOOR TO PREVENT BEING IN RAIL RUNNER WHEN IT STRUCK ANOTH ER EMPTY PARKED RAIL RUNNER. THE S& S SCOOP WAS POSITIONED NEAR THE TRACK. HIS ELBOW REPORTEDLY STRUCK BUCKET.
AS EE WAS PULLING A PIECE OF LOW STRUCTURE WITH THE SCOOP, A CHAIN HOOK UNEXPECTEDLY CAME LOOSE AND THE AIRBORNE HOOK CHAIN SLAPPED BACK IN THE DIRECTION OF THE SCOOP AND THE HOOK HIT EE IN RI GHT EYEBROW/RT. CHEEK. HOOK CAUSED LACERATION THAT WAS CLOSED BY SUTURES. THE CHAIN WAS ON A CM PULLER.
ACCORDING TO INJURED HE HAD JUST DUMPED A LOAD OF COAL FROM HIS CAR INTO THE SECTION FEEDER AND AS HE DROVE AWAY HE HIT INTO A HOLE IN THE ROADWAY AND THE BOUNCE RESULTED IN BACK PAIN. THIS RE SULTED IN A DR. VISIT AND DR. SUGGESTED TIME OFFBECAUSE OF STRIN LIGAMENT POSSIBILITY.
ACCORDING TO INFORMATION GATHERED FROM HIM THE PUMP MOTOR WAS GIVING OFF AN ODOR AND WAS HOT. AFTER APPROX. 15 MIN. EXPOSURE HE BECAME NAUSEATED AND PARKED THE SCOOP. EE WAS TAKEN TO DR. FOR E XAMINATION DIAGNOSIS-SMOKE INHALATION.
WAS SAID TO BE RIDING IN ON MANTRIP ABRASION WAS CAUSED BY HEAD SCRAPING AGAINST MINE ROOF WAS WEARING HARD HAT & SAFETY GLASSES.
I NAME STATED THAT WHILE TAKING THE PERSNNEL TRIP TO THE FACE HE RAN OVER A PIECE OF DRAW ROCK IN THE #3 ENTRY TWO BREAKS OUTBY THE FACE BOUNCING HIM IN THE DECK OF THE SCOOP CAUSING PAIN IN L OWER BACK
WHILE DESCENDING HILL, FOR SOME UNKNOWN REASON, VEHICLE SUDDENLY ACCELERATED BEYOND SPEED DRIVERCOULD CONTROL. VEHICLE STRUCK BERM ADN FLIPPED OVER (TRUCK WAS LOADED) TRUCK HAS SEAT BELTS & THEY WERE NOT IN USE.
WAS DUMPING LOAD OF COAL AND WHEN BED ELEVATED THE HOIST PUSHED THROUGH THE BED OF TRUCK AND LOADED BED FELL BACK DOWN CAUSING A SEVERE BOUNCED AS BED STRUCK FRAME APPROX. 20 TONS WITH 4-5 FT DROP.
